I'd raise the epap to 7, maybe 8

I don't like the 5.5 PS myself.

How many times do you think you woke up during last night?
I see one break in therapy where I know you were awake...why?

Thanks, guys. I will try that, Pale. Pugsy, you agree I need to raise the EPAP? I’m not sure what you mean by you don’t like it either.

And leave the top end at 11?

I have a lot of problems falling asleep. I can lay there and sort of drift in and out but sometimes I give up and just lay here for a while. Last night I felt like I stayed asleep one I managed to fall asleep. But I don’t ever feel refreshed when I wake up.

I don't like a high PS ...your 5.5 PS
I would rather see you raise the EPAP a little and lower the PS to maybe 4.
Now you might really like that PS but for me I found it tended to make me feel like I was trying to hyperventilate...wasn't comfortable...felt weird.

Try PS of 4 or 4.5 one night and see how it feels...
PS of 4 or sometimes 5...the most commonly used PS when someone is put on a bilevel machine like yours or even the auto bilevel.

PS...pressure support..the difference between EPAP and IPAP.

So if you increase EPAP by 1 cm...that would be 7 EPAP...and yes leave the top end alone...that is 11.5 which then would give you PS of 4.5 and I like that a lot better than 5.5.

I'd agree.

You also might try 400mg magnesium glycinate before bed. It seems to help me, or maybe I'm just imagining it.

I changed the EPAP pressure up to 7.0 and no real change for the better. I will try increasing it to 7.5 or 8 tonight. Any thoughts are appreciated.

Thanks.

If those events are SWJ then pressure changes isn't going to help.
You probably need to do the crash course in figuring out if the events are real and you are asleep or if they are SWJ and not real.
Watch all the videos here

Make sure that what you are wanting to fix with more pressure is fixable with more pressure.

I have watched them a number of times. A lot of my centrals appear to be real but I've been doing this for 8-9 months so I wouldn't rust my eye to be honest. I wish there was a service I could send my SH data to for an evaluation so I could get a 2nd opinion to compare to the one from my sleep PA. Cause at this point I am really confused and not sure what to do.

How many is "a lot"?
Some central flags are normal and no big deal. We all can have them...sleep onset centrals are normal to see.
I am betting that those flagged right when you turned the machine on at 00:20 that first little cluster...aren't real.
Along with the ones very close to the other known awake times. If you remove those from the central count...how many are maybe real?

If I disregard the 4 that occurred between 00:20 and 00:23 (which were labeled as 4 CAs) I still had between 23 and 27 CAs plus 12 OA events. I don't want to overwhelm the board with images but I took zoomed in shots of each of the CA events so I can post some of them if you want to see. Of course, I may have zoomed in too far on them. But most of the events seem to have the fairly flat lines before and after each event that the video indicates is a true event.

Sorry to be such a pain with this. Let me know if you want me to upload some of the images.

Thank you again for your patience.

Try more pressure...see if it helps or not. All I am saying is that more pressure isn't going to fix the SWJ centrals and there's more to determining SWJ than the flat line before and after the flag. There's evaluating the flow rate for a minute or two or even 3 prior to the flagged event as well as afterwards especially when I know you turned the machine off.
You had more than those first 4 centrals that were probably SWJ last night. I see at least 2 breaks in therapy so I know you woke up at least twice because you turned the machine off.

You can't fix/prevent centrals with this machine anyway...best you can do is reduce the obstructive stuff a bit and see if the centrals reduce and they might if those centrals are related to obstructive stuff causing an arousal.

You can upload it to https://www.dropbox.com/request/0LalWvEsijRIgX8sdM7H and I can take a look.

Thanks, PR! I didn't get on my computer last night but I uploaded my SD card via a Zip folder this morning. Hope that is the correct way to do this. If not let me know. Any thoughts you have are appreciated.

BTW, the night of July 17th was the first night I used the PR Dreamstation Bi-PAP Pro having been switched over from the PR Dreamstation APAP.
