I need to know about currents medications and how was your sleep quality for last night?
Did you sleep soundly or have lots of wake ups last night?

I take 15mg Lexapro daily, Claritin daily, multi vitamin daily, magnesium supplement (for last 3 mos) and OTC sleep aid (doxlyamine which is a Unisom generic) I take 2 each night at bedtime, also Flonase nasal spray each night to help w nasal stuffiness due to nasal allergies like dust, pollen etc. I also omit caffeine after 3pm.
Sleep last nite: a lil better than most nights. Generally speaking ,most nights ,I toss and turn after initially falling asleep. I change positions frequently for unknown reasons. Last night I felt like I tossed LESS than usual.
Thanks

Read up on the side effects that come with Lexapro.
It's not doing you any favors in the daytime energy department. Fatigue is a well known side effect.
https://www.rxlist.com/lexapro-side-eff ... center.htm
Plus it is a SSRI class medication and those meds also affect sleep architecture and mess with the sleep stages.
Google "SSRI and sleep" and start reading.

So part of your unwanted symptoms could be medication side effect related.

All that said...your AHI isn't all that great either but about half the AHI is central (those Clear Airway events) in nature and half is obstructive (OAs and hyponeas) in nature.

It wouldn't be impossible for the CAs/centrals to be post arousal events meaning maybe more related to the OA/hyponea causing an arousal and then the arousal causing SWJ flagged breathing CA. It's a stretch and going out on a long skinny limb for sure.

You could try more minimum pressure and see if the OAs reduce....and then if they do then see if the CAs reduce.
I am thinking something along the lines of 8 or 9 cm as a minimum pressure.
It's a guess though....I don't know how much of what you are seeing is related to medication related arousal and SWJ and I don't know if you have the skills to try to figure out awake vs asleep breathing which is what really needs to be done here.
Watch the last video on this page to get an idea what I am talking about.

Regarding pressure settings-As of 2 weeks ago the doctor has it set in a range of 9-17. Over the last six months it has been adjusted a couple of times by the doctor,.Previous setting range was 8-15.
also I failed to mention I had a titrated sleep study in April 2018. It was a terrible night of sleep it took me 2.5hrs it seemed to fall asleep . Literally I felt like I slept maybe three hours in actuality I think I slept about four hours based on the report . I couldn’t get comfortable in the new environment . Anyway based on THAT nights data, dr. seems to think that 10.4 is my optimal pressure .

I use the ramp feature- should I use the ramp feature? It usually takes me about 30 minutes to fall sleep .

Will do some reading on the SSRI and sleep later tonight.
Meanwhile I have a call into the doctor I’m going to ask her to change the min setting. I requested 9.5 as another helpful members suggested .
Have a good day .

I am one of the few that uses ramp and finds it a valuable comfort feature.

The key is to have the start pressure high enough that you don't feel starved for air, and to set the ramp time short enough that you transition but don't linger at the lower pressures.

My minimum pressure is 11. My ramp starts at 8 and I found that 10 minutes is all I need to adjust to my minimum pressure. It doesn't matter if I am awake of asleep when I get to my minimum pressure. What does matter is that the pressure transitioned smoothly over a period of time.
