CPAP effect wearing off?

Post by tchan » Thu Jan 25, 2018 6:33 am

Hey guys, I've been on CPAP for a month now. I have an AHI of 57 and when I first started my treatment, the first two nights I felt amazing after waking up. However, after that, I can't really seem to tell the difference anymore and am still tired when i wake up.

Has anyone else been through this and can offer any advice? On my treatment I have been consistently getting <5 AHI per night.

Unless you take the time to fill in your equipment profile and show some Sleepyhead charts, it's difficult to help. Go to the Control Panel. Then download Sleepyhead. https://sleep.tnet.com/equipment

So how many hours of sleep are you getting?
Are those hours fragmented by many wake ups?
Do you take any medications and if so, what?

And yes...I have heard of it before and we not so affectionately say the honeymoon is over. Normally the honeymoon lasts more than 2 days though.

But let's make sure that there's not something on those detailed reports that is pointing to maybe suboptimal therapy.
See this thread as to what we are talking about.

If you use SleepyHead....don't do the full screen toggle thing because there's a bug in SH that involves some computer operating systems and you get full screen and can't easily get out.

Yes, I had an initial energy boost, then felt more normal. Part of that is untreated your body uses a lot of energy / adrenaline etc at night. so when you first get treated that energy your body planned to use at night goes unused... so your feel fantastic... Then your body realizes it doesn't need it anymore and that stops happening...but, your body has also been trained to conserve energy during the day in order to survive the night, and your body has to normalize the endorcrine system... heal a possibly huge amount of brain damage (up to a year of healing for severe sufferers), and all other sorts of normalization. My energy waxed and waned for a long time... 11 months later I'm feeling fantastic, and most of the time better then the first day I put the CPAP on... and definitely smarter. so hang in there! Exercise, plenty of omega 3's and getting the cpap working well all help.
Instead of Sleep apnea it should be called "Sleep deprivation, starving of oxygen, being poisoned by high CO2 levels, damaging the body and brain while it's supposed to be healing so that you constantly get worse and can never get healthy Apnea"

Like nicholas says, our brains produce extra adrenaline to keep us alive in spite of apnea.
After a period of time, the supply decreases, due to diminished need.
And, doggone it; that adrenaline feels awesome!
The letdown you feel will level off in a few days, and you will feel less deprived,
and eventually begin feeling "normal". Normal is pretty good.

There are a lot of problems involved when you have OSA (specially severe). Most of them are going to be solved in the long-term, but they are probably the most important.
White brain cells damage (which leads specially to concentration and memory problems), hormonal problems, stress and other mood related problems, physical performance and resistance and many others. You should be relieved that after the first nights you felt awesome, that means your therapy helped. The letdown that nicholas and chunkyfrog mentioned might be the reason why you are not feeling that great, but soon it will be over (and normal will be excelent).
However, you should always use SleepyHead to monitor your last night and see if there's any improvement that could get you to the optimal treatment.

Very good point Lucas! Yes, I see now that it might have looked like to someone new that I was suggesting to ride it out, but Yes! By all means optimize treatment. Personally it took me 9 months to completly optimize things (perfect mask, perfect chin strap, perfect mouth equipment) But some may figure out what works best earlier!

Hey all, thanks for the helpful information.

I've been trying to get my data imported onto Sleepyhead with zero luck, both on windows and mac. It finds the correct file path but when I click import it just does nothing.

My machine is the F&P Sleepstyle Auto which doesn't seem to be under the machines that Sleepyhead supports...

Any other ideas?

What are your pressure settings? Too often MDs think leaving the min. pressure at e.g. 4 (machine's default low) will allow it to address any events high up on the scale, but it can't reach there in time and we need to bump the setting at least to more manageable places like 6 or 7 to be therapeutic at all. Does this sound anything like a possibility in your case? The high setting is usually fine to leave open even at 20, though e.g. 15 would not be terrible... but it's definitely the low (min) one that does the work.

Does anyone know if InfoSmart ( https://software.fphcare.com/Download/D ... dee35baebb ) will work with SleepStyle Autos?

Is it model 254?

I would get a ResMed or Phillips latest model Auto. (ResMed S9 AutoSet is also good. Some good, used ones are available.)

It appears so. Down near the bottom of the page. Assuming he doesn't have a 10 yr old version of this model and instead has this latest model.
https://www.fphcare.co.nz/homecare/slee ... pstyle-en/
Apparently also has some sort of phone app...probably all you get is like what ResMed does with MyAir but a little is better than nothing.

To the OP....

Actually just because SleepyHead may not list it...I would at least try it. If the file structures aren't all that different from the F & P Icon (which SH does support) it might just work.

Usually when SH is not compatible with something you get a message "not compatible" "not supported"
When SH just sits there usually there's a problem with the import and SH isn't even getting the files to be able to figure out if it can read them or not.

But you can use InfoSmart ....it's a PITA to install and use. So Fair warning....
