That sounds like your OSA is highly positional (Much worse on back as compared to sides or stomach.).
Mine is like that. If I stay off my back, I can use a moderate pressure to achieve low AHI. Or, I can wear the firm foam cervical collar and achieve the same results in any position. I opt for the collar and sleep on my back most of the night.
