Hybrid and ResMed S8 Elite
I've gotten the Hybrid configured so it seems to be working pretty good for me, but I get terrible numbers from the machine in the morning.
Using my Aptiva mask my HI runs maybe 3.0 or so ... using the Hybrid ( with the machine still set on Activa ) my HI this morning was 13.1.
But I feel good and when I wake up I feel like I am sleeping good.
It doesn't seem to make any difference which setting on the machine I use. I tried the MIR Full and got terrible numbers there too.
I can't imagine that changing masks would actually cause me to have so many more AI's and HI's. Especially since I don't change the pressure.
If you are using a Hybrid mask on a Resmed S8 machine, what settings are you using for the mask?

Your HI numbers are much less important than your AI numbers ... if you are getting low AI numbers and you are feeling good, then your treatment may be working.

Actually the AI numbers are high too.
With the Activa mask the AI runs 1.something up to 3. something usually.
With the Hybrid this morning is was over 7!
But I'm still getting air up my nose at the same pressure.
So why do the numbers get so bad?

Are your settings right? Seems to me that I remember reading that there was something about setting up a Resmed machine for a hybrid. Can't exactly remember what though. Thought that it had something to do with the mask setting... would that impact the AHI though?

Looks like you have the software ... look at your nightly data and see if your leak rate is going off the chart after you go to sleep. That is what mine has been doing. During the night, as you toss your head around there is a good chance you are developing large leaks. The machine then tries to compensate by increasing pressure ... however, the pressure may not be enough to overcome the leaks and you get apneas.Arizona-Willie wrote:Actually the AI numbers are high too.

You have to make certain you have resolved all significant leak issues before you can begin to investigate your pressure setting. In any case, you want to shoot for an AHI under 5.

Hybrids and ResMeds
I honestly believe that Hybrids are a BAd match with a ResMed. At least the original Hybrids before the exhaust modification. However, some have reporetd good results with a ResMed and the Hybrid. I'd correspond with MikeMoran or Bookbear for more info, if I were you. They seem to work fine with REMstars though. However, I have NO direct experience with this. I sold my Hybrid after three unsuccessful hours of trying to get it to work with my S8 AutoSet Vantage. I've never tried it with my REMstar Auto, tho many here have with good success.

Exhaust modification?
What is the exhaust modification. Somehow I haven't seen info about that.
Can we do it ourselves? How do we know if our mask needs it?
I got mine from my DME so it might be an older model that needs it, dunno.
I know it has the cheesy straps.

The Hybrid with its 50 LPM @ 10 cm/H2O passive exhaust flow may not
be a good match for the ResMed. However, with a couple of pieces of tape
you can fool the ResMed into thinking its a UMFF.
Remember the ResMeds responds more aggressively to "events" than the
RemStars. Therefore, in auto mode, the rapidly advancing pressure may
be creating huge leaks for your interface. Also, don't forget these machines are not really marking "true" AIs, like a PSG, they are marking their own flow limitations and snores from a microphone. Pulling the hose across the
headboard may mark a snore.
If you are at a high pressure, say 15 cm/H2O, is it conceivable the AIs
being marked are just the flow limitations that occur when the APAP runs
out of air?

Modify exhaust?
My pressure is set at 9.6 right now I believe. With EPR set on 2 I get a reported 8.8 or 9.0 most of the time. It varies for some unknown reason, especially with the Hybrid.
How do you fool the S8 Elite into thinking it is a UMFF?
Mine is the Elite and it is straight CPAP, no Auto.

Try calling the Hybrid people at InnoMed 800-200-9842, they are usually very friendly and helpful.
Ask them what mask settng you should be using on the S8 Elite ... maybe they have already found a solution for the problem?

I strongly suspect there is a problem with the high exhaust rate and the S8.
In the wee hours this morning I noticed that the S8 seemed to be out of synch with my breathing. It was either fast or slow, not sure which. But it seemed like the pressure was increasing before I was starting to breathe in.
