No consistency with numbers

Post by lennyten » Sun Sep 17, 2006 6:53 am

My numbers go all over the board. What would cause higher AHI, even thou the leak rate is lower?

Example:

AHI - 6.6, AH - .3, HI - 6.3, Time in Apnea - .1%, Leak - .70

Then, the next night

AHI - 12.2, AH - 3.0, HI - 9.2, Time in Apnea - 1.2%, Leak - .46

Thanks.

What's your pressure setting?
Are you running a range or single pressure?
Is the mask setting on your machine set correctly?

(5) REMstar Autos w/C-Flex & (6) REMstar Pro 2 CPAPs w/C-Flex - Pressure Setting = 14 cm.
"Passover" Humidification - ResMed Ultra Mirage FF - Encore Pro w/Card Reader & MyEncore software - Chiroflow pillow

Pressure is 15.0 - CPAP, with ERP set at 2.

I have tried APAP and CPAP, with different settings, as prescribed by my sleep doctor. Have never achieved AHI less than 4.0.

lennyten,

As you've changed your pressure settings (I assume 1 cm at a time), do the results change as you go one way or the other?
How far below or above 15 have you gone?
Does 15 seem to be about the best you can get it? (so far)

Do you have any other medical issues that may be affecting it? (RLS, PLMD, whatever)

Maybe some of the others have some ideas.....

Best wishes,

What I am trying to understand is what would cause the AHI to be 6.6 one night, and then 12.2 the next night, with the exact same settings? And, as mentioned, the leak rate was lower on the night with the higher AHI?

Thanks.

One thing that jumps to my mind is the possibility of untreated or undertreated GERD (acid reflux.) Even if a person has no noticeable symptoms, there's such a thing as "Silent GERD."

There've been some interesting topics about how acid reflux can interfere with any kind of cpap treatment -- more so on some nights than others for no apparent reason:

First of all, you only provide two sleep sessions of data ... is that what you are referencing your observations of "all over the place" on?

It may be that you spent the first night mostly on your side and the second mostly on your back?

I think you need to collect more data before trying to determine what the problem is if indeed there is one regarding the PAP treatment.

I think you have hit on my problem. I have been diagnosed with acid reflux, but never have any pain, etc. I eat way too late at night, am a little overweight, etc.

Restedgal, I really appreciate your help. I am going to definitely eat earlier, start taking Nexium again, and it will probably also help me lose weight.

Along with Rested Gal's, suggestion, are you mouth taping, it looks like your bad numbers also follow leaks. If you don't keep a consistent low leak rate, your numbers will run wild. Jim
