Does this sound like Central Apnea

Post by Justin_Case » Thu Aug 24, 2006 1:25 pm

Although two poly tests showed nothing as far as CA(so I have been told) but in reality, I have no idea if it was even looked for, I find myself when trying to go to sleep (last night was particularly bad) that I get subtle jolts or shocks in my head when trying to fall asleep. I am by now means a strong sleeper, in fact, I am very light. Most of the time, usually in the mornings I am trying to fall asleep and I can hear the birds and surrounding noise, but my CPAP rams up sensing events. So, even though it may seem that I am not asleep, I may be in Stage 1.

I also have a dental (Klearway) appliance and overall, I find it does nothing to help my sleep apnea so I am using APAP; however, what I do find it does help keep this type of jolt or arousal to a minimum.

Any ideas?

sounds like you may need to increase your Minimum (Min.) pressure. If it is at the default 4cm that could be your problem. You would need to increase it to 6cm or so.

What was your cpap pressure (cm) found during your last PSG?

One type of those jerks are called hypnic jerk. Your leg may also twitch with this jerk but it is completely normal. It is normal to have 1 or 2 of these per night as you transistion from awake to sleep.

But if you are having more than that, it probably means you are having SDB events which is preventing you from reaching deeper sleep. In that case the cpap pressure should be high enough to prevent those and allow you to transistion from awake to sleep jerk free.

I was thinking about that.

Originally, without mask or titration (my sleep doc was very good in the amount of pressure) he prescribed 10 cm. In my second subsequent study with the mask, I had to go up to 12 to stop snoring.

The last few nights, the minimum has been 8 which was fine the other night, but last night no. I then went and used my dental appliance. I did feel my leg twitch once. I noticed the pressure did climb to 11 for several minutes unlike the night before, where I was at 9 most of the night.

Tonight, I think I will increase the pressure to 8.5 without the appliance. If I have the twitches I will use the appliance.

Funny thing... my pressure peaks during the late morning and I don't feel I am in REM either (no dreaming) and I am almost fully aware, just lying in bed waiting to sleep.
