sleepy head data

Post by TGtired » Wed May 10, 2017 10:03 am

i am about a month into using my APAP for mild apnea with AHI around 6.5 and UARS. I am finding lately all my events are clear airway and it seems i am waking up alot with the apap. I guess my question is, is this normal and is the clear airway possible waking me up.

It's possible that the Clear Airway/Central events are waking you up.
It's also possible the the wake ups are responsible for the Clear Airway/Central flagged events....now whatever is causing the wake ups is up for discussion. Those CAs could be SWJ (sleep/wake/junk).

Did you have a sleep study done in a lab to determine pressure settings needed or were you just given an apap machine with it set to a range to let the machine sort it out?
Some people who are put on cpap therapy will develop centrals as a result of the cpap pressures (can be high or low pressures) but that is usually caught with the in lab titration sleep study.

Maybe if we could see what you are seeing we could offer better ideas.

i had a sleep study a year ago with a titration and I was very mild at that point. The cpap and bipap caused centrals. so I got an ASV and never could tolerate it. The sleep lab was horrible with follow ups . So over a year later I went to another place didn't get enough data on a sleep study so they tried to bring me in a second time and insurance denied it. I ended up doing home study which showed mostly hypopneas and just a little obstructive and AHI of 6.5. So right now I have the sleep labs APAP to try out. I was told by them I should have never been given an ASV. Long story but sleep labs where I live stink and the main doc at one doesn't even believe that hypopneas cause an issue. So it seems like I am on my own.

Well......if cpap/bipap causes centrals where no centrals existed prior to pap therapy then ASV is certainly worth considering so I don't know why you were told you "never should have been given ASV" because sometimes it is indeed what needs to be done.

Have you seen this video about complex sleep apnea which is what a person has when they have both regular OSA (obstructive/hyponea apnea events) and central apnea events (no matter what the cause)?

https://www.youtube.com/watch?v=CU-XTcf ... e=youtu.be

Now sometimes centrals caused by cpap/bipap therapy will go away after a period of time of being on cpap. It does/can take a long time though for that to happen...talking months here.
How long are you planning on using the apap provided for trial?

Feel up to digging deep into your flow rate to try to figure out if those centrals are real or not that you are seeing?
Normally I wouldn't go down that road so early in someone's therapy but with your history of cpap actually causing centrals then we probably should look at yours and see if you are asleep or not when they are getting flagged. If they are SWJ centrals then we shrug our shoulders because they aren't real but if they are the real deal then we could still keep a close eye on them while giving it time in hopes that your body adjusts and no longer responds with the unstable breathing that the centrals represent.
It's possible that they could go away with some time but if you are waking often and sleep is horribly fragmented...makes for some really ugly sleep quality nights.....Just trading one sleep disruption (the mild OSA/UARS) for another sleep disruption (complex sleep apnea).

well I have one lab who seems to only use APAP and BIPAP and are easy to deal with. They wouldn't even talk to me about my ASV. The other place only wanted to work with ASV and were just horrible to work with. I would try the ASV since I have one but I have no clue how to set it. This might be a stupid question but how to I post the sleepy head data so you can see it?

How to post images of your software reports.

I prefer imgur myself but you can use whomever you wish.
https://sleep.tnet.com/reference/tips/imgur

But we don't need all the graphs that you can see...examples of what we usually like to see
viewtopic/t103468/Need-help-with-screen-shots.html

How to organize the graphs
https://sleep.tnet.com/resources/sleepyhead/shorganize

Which ASV machine brand/model do you have?

Was the UARS diagnosis more of a "well your AHI is mainly hyponea so you probably have UARS" statement or did they really do a sleep study with PES to for sure measure airway patency?

UARS is hard to diagnosis and even harder to treat because the usual markers for "good therapy" don't necessarily help us all that much with UARS therapy evaluation.
Doubly hard when the cpap pressure is causing centrals and you probably need more pressure....talk about being caught between a rock and a hard place.

I will try to post my data when I get the Imgur figured out. The ASV is a resmed Air curve 10

Nice machine. I am a bit pea green with envy.

I have some thoughts but short on time right now to compose anything.
I am going to need some more information most likely too....when I have more time I will make a list of questions.
Your situation is more complicated than plain jane vanilla OSA.

Oh...one question for sure right now...on any of your sleep studies without cpap of some sort what were the oxygen level drops?
Hyponeas can be just as ugly as full grown OAs if you have enough of them that are prolonged stacked one on top of each other...so what was the lowest oxygen level reported?

Just saw your first image...get off the "events" tab and go back to the "details" tab.
Omit masked pressure and see if you can include leak graph and the flow limitation graph.

You can turn off the pie chart in SleepyHead....Preferences/Appearance tab...remove the check mark for "show pie chart"...though that's not critical as I can see the statistics well enough.

Don't need 2 pressure graphs....don't need mask pressure and regular pressure graphs both...pick one and turn off the other.
Do need the leak graph and maybe the flow limitation graph because flow limitations could be causing those pressure changes you are seeing.

Don't worry too much right now about getting those just perfect.
I have one thing I would like for you to try to do tonight....if you wake up just reach over and turn the machine off and then right back on again.
I want to see just how many wake ups you might be having and are they happening right after a flagged event of some sort or right before.
Plus it will let us better evaluate sleep vs SWJ status if we have the little markers in the therapy line where you turn the machine off and then right back on again.

I doubt your leak graph is going to be very exciting and the FL graph probably won't be either but need to see it.
You can wait until tomorrow if you wish because I want to see if the pressure increases in relation to FLs are also happening at the same time as a wake up.

i took several screen shots i have no idea how to turn on and off things yet so hope i don't bombard you with this. I did turn off my machine the first time when i woke up. I was up for a bit so but the machine back on like an hour later or something. I will post screen shots in a bit.
