I saw the picture and the multiple holes. To me it looked like something too hot had melted the plastic or caused the plastic to disintegrate or "rot". For the most part the holes were pretty much round or almost round. There was a whole bunch of holes too...it wasn't just one little hole or tear.

OP never answered the question ... how often and what do you clean it with ?
looks to me like the plastic was dissolved or softened by something from the inside.
If you're not part of the solution you're just scumming up the bottom of the beaker!
