evaluation please

Post by Winlocktim » Mon Jul 11, 2016 7:44 pm

Hello, I am new and not under a doctors management.

I was hoping that someone that knows what they are doing can look at the settings and give me an idea on if this is correct or should I adjust settings. Any ideas would be a big help.

I can see the picture. You have some large leaks going on. Pressure is set at 4-11. A minimum of 4 is really low. You have a mess going on from 3-3:30am and 5-6:20am. Were you awake at those times? Both times have flow limitations. You are maxing out the pressure a lot. I don't know enough to advise you what to do.

Can you fill out your equipment profile in the user control panel?

I can't enlarge the image enough to see the smaller numbers but it appears that your minimum and maximum pressure settings need some adjustment.
I would suggest increasing both the minimum and maximum by 2 cm and see what the reports look like then.

I don't see any large leak flags on the events graph. I see the big spikes but that's probably mask adjustment and not long enough in duration to matter.
If large leaks were present they would be flagged on the Events graph...near the top where it says LL...that's large leak.

I was thinking that I need to bring the pressure up some.. It looked to me that I maxed it out from time to time.

I think the "mess" was a leak around the upper part of the mask I was thinking of strapping the puppy on much tighter tonight.

Is that AHI reporting something that the machine could or couldn't control?

OK, large leak is .11%. What does the flow limitation indicate?

The minimum and maximum pressure need to be increased. Personally, I would increase Minimum from 4 to 7 and the maximum from 11 to 15. (minimum is the most important). Try that for a few days and see if your AHI doesn't drop below 5.

do it like this next time: https://sleep.tnet.com/resources/sleepyhead/shorganize

don't shrink the pic, it makes it hard, or impossible to read.

your min and max pressures are too low.

I used the Quattro FX for about a week. It doesn't need to be smashed down on your face. That will hurt the bridge of your nose. Once you start the air flow sort of readjust the mask so the silicone right next to your face fills with air. Wash the mask in mild soapy water and wash your face just before bed so you get a good seal. If that doesn't help you might need a different mask.

or a mask liner, huge game changer for me when I started using a FFM years ago, it went from "driving me crazy" to "well, yeah, sure, I can live with this".

Not exactly sure what you are asking here but it sounds like you are wondering if the AHI you see is what the machine prevented or let slip past the defenses.

The AHI is the apnea events that the machine wasn't able to deal with for whatever reason (and in this case the pressures aren't quite optimal)...they slipped past the defenses.
We have no way at all to know how many apnea events were actually prevented.

OK, wasn't sure if flow limitation was something with the hose. So increasing the pressure will help air flow.

Tim, centrals can't be helped with APAP but your Obstructive and hypopneas should get better. So should the snoring.

0.11 % time in large leak..for anyone isn't bad at all and especially it's not bad for someone just starting this therapy.
I can pretty much guarantee that I had more time than 0.11% of the night in large leak territory last night.
I don't even give it a second glance until I hit double digits and then I look to see how far I went along with prolonged segments vs short spikes.
