Each Advil® PM caplet contains 200 mg of ibuprofen and 38 mg of diphenhydramine citrate

Ibuprofen can increase your risk of fatal heart attack or stroke, especially if you use it long term or take high doses, or if you have heart disease. Do not use this medicine just before or after heart bypass surgery (coronary artery bypass graft, or CABG).
ibuprofen may also cause stomach or intestinal bleeding, which can be fatal. These conditions can occur without warning while you are using ibuprofen, especially in older adults.
Do not take more than your recommended dose. An ibuprofen overdose can damage your stomach or intestines. Use only the smallest amount of medication needed to get relief from your pain, swelling, or fever.

Nervous system side effects have been reported frequently. These have included depression with drowsiness and sedation in nearly all patients treated. Motor skills may be impaired. Dystonic reactions have been reported after single doses of diphenhydramine.[Ref]
Cardiovascular side effects have included hypotension, tachycardia, and palpitations.

But in the end, it's effectively an aspirin, and unless you have a known ulcer or other problems already known of when you take them, while I wouldn't use them every night at all, one, once in a while is unlikely to kill you.

Ibuprofen, and many other NSAIDs, act in a similar fashion as aspirin, but they carry much higher risks. While aspirin has heart protective qualities when taken at lower doses, these newer pain relievers do not provide that same level of protection. In fact, just the opposite. FDA strengthens warning that NSAIDs increase heart attack and stroke risk
Examples of commonly used over-the-counter NSAIDs include ibuprofen (Motrin, Advil) and naproxen (Aleve); celecoxib (Celebrex), diclofenac (Cataflam, Voltaren) are prescription NSAIDs. Aspirin is also an NSAID, but it does not pose a risk of heart attack or stroke and is not covered by this new warning.
•Heart attack and stroke risk increase even with short-term use, and the risk may begin within a few weeks of starting to take an NSAID.
