New APAP machine - 4 nights
After 5 years with a straight CPAP machine, I have just purchased an APAP (humidifier is still on it's way, but that is a long story. My prescribed pressure on the CPAP was 10 cm, although I had reduced that to 9.5, and I had turned off C-Flex because I felt like it wasn't doing much good, and may actually have made my AHI slightly worse. The prescribed pressure range for the APAP is 8-12. With A-Flex on 2, for 2 nights my AHI was 1.89, and for 2 nights on 1 was 1.56. Last night, the minimum pressure was 8.00, the weighted average was 8.09, 95% was 9.40, and the maximum was 9.50. A lot of the time the pressure has been stuck at 8, and the maximum pressure has never been above 10.80 (very briefly). I am not using any ramp pressure. My question is: does this seem like I might need to reduce the pressure to 7-11, and if not why not? Is there anything else that I should be monitoring before I do make the pressure reduction? The machine is very quiet, and I feel like once I get it dialed in correctly, I am going to be loving it! Already it is supporting my feeling that I may have some times when I need the extra pressure, although the pressure spikes seem to be associated with leaks - movement dislodging the mask, perhaps?

You can make changes and observe the differences in auto pressure and AHI that result. It's certainly not going to hurt anything. I recently did something similar, and found that I had a significant increase in AHI at the lower pressure. YMMV. I had dropped from a minimum of 9 to 8, and returned that to 8.5 and we'll see what that does after a week. Good luck!

Really, 4 nights does not a trend make. (I think I just channeled Yoda??) AHI seems good, I would let it ride a week or so if it is otherwise comfortable.

Thanks for the replies. I guess it's a bit like almost every new toy I had as a kid - I couldn't wait to fiddle with it to see how it worked! The idea of a week sounds reasonable - especially as I should have the humidifier by then. Weather is all over the place at the moment so varying humidity may be playing a part in variations...
