Newly Diagnosed Need help with report
I have been lurking around a few days and have learned a lot. This is a great place with a bunch of great folks.
I have a copy of my sleep study report and would like some help understanding the results please.
I am a 40 year old male. The sleep study came about because of me telling the doctor how tired I was all the time. I thought I was just getting old until they found the apnea.
I am fixed up with the latest and greatest (I think) machine and stuff. I am still swapping masks around and doing pretty well I guess.
Here are some numbers off the report. If I leave anything out, let me know what, and I'll post it.
WITHOUT CPAP
sleep time 124 mins
sleep efficiency 56.4%
84 mixed apneas
0 central apneas
hyponeas 49
AHI 64.4 per hour
oxygen desaturation index 18.8
minimum o2 saturation 52%
"The findings are consistent with severe sleep apnea"
They woke me up and put on CPAP:
Baseline 02 saturation 98%
"CPAP titration was initialized using the settings of 5 cmH20 through 14cm H20. The setting of 14 cm H20 was adequate to control respiratory disturbances."
sleep time 253.0 mins
"Persistent oxygen desaturation was noted after the titration. The saturation was less than 88% during 96 minutes"
No parasomnia was noted
ECG displayed normal sinus rhythm.
PRESCRIPTION:
AHI 64.4/hr
Nadir 02 52%
Obstructive Apneas 84
total apneas 12
"lifetime" checked
CPAP 14 cmH20
02 supplementation 2.0 LPM via bleed in
Heated humidifier recomended
"Mask of choice"
If there is more, just ask. I have a folder full of stuff.
I'm thinking this Apnea was pretty serious. I feel like I did when I was in my teens! Even the day after the sleep test, I just felt great. Other than throwing mask off in the night a couple times, I guess I'm tolerating the treatment well.
(If anyone has trouble taking mask off in the night, try some tube socks over your hands. That broke me of that. A few nights later, I could sleep without them)

Just for the record, what are your pressure settings? And exactly what machine & model # do you have (and what type of mask)?

More important now is how you are doing with your machine...once we know which machine you are using then hopefully it is the latest and greatest with software available so you can monitor your therapy if you want....

I have the ramp feature set to start at 9cm
Im doing ok, I guess, I just need to settle on the mask. The full face mask is a little bothersome, but works well. I prefer the nose pillows, but my mouth is dry by morning. I have tried a chin strap to no avail.
The only other problem would be the oxygen machine makes a ton of noise. I guess I need to move it to another room or something. The Dr. wants me to use it a couple months and see if i still need it. I guess I need to buy one of those recording 02 sensors that records to go on my finger overnight and see how I do with and without 02 bleed.
I'd sure like to dump the oxygen machine if possible.
Not sure why I need it. My 02 level while awake is 98%.. Any ideas?

I don't think that's a record, but you're certainly in the running for worst o2 sats!minimum o2 saturation 52%

Your machine works well with SleepyHead software.
I think your doctor probably doesn't want to risk those really low O2 levels at night while you are adjusting to cpap therapy. Hopefully once your therapy is optimized and O2 levels no longer plummet (confirmed with follow up overnight pulse ox readings while you use the machine) you won't need the added O2.
At your lowest levels...I sure wouldn't be cheating by sleeping without the cpap machine. Your level was scary low.
You can get your own overnight pulse ox if you wish and that way you can follow it yourself. Not horribly expensive either.

Some will also work with Sleepyhead. Check with other forum members as to which ones work best. I don't keep up with that aspect of the various models.
Your doctor can also have your DME do some overnight pulse ox tests if you don't want to buy your own unit.

Justin this is most likely why the doc wants the O2 supplementation. Until you get settled with CPAP and see a significant reduction in AHI, the div wants to make sure you're not losing too many more brain cells due to the extreme desaturation you've evidenced on your sleep study. I'm guessing this was what was making me feel so poorly/tired prior to CPAP therapy?
I will not take a nap without the machine. In fact, I haven't been needing naps anymore.
I did buy another machine just like I have to travel. The company I got my main machine cut me a sweet cash deal on a second machine, less the heated hose option. I travel about 10 days a month and it is nice to keep one machine paced and ready. They also put one of those cell phone things on it like the one my insurance co. bought so they could "gather compliance data" I guess the insurance co. wants to know you are using the machine.
The o2 machine they gave me is huge and loud. I prefer not to travel with it. Any thoughts?
I will go ahead and buy one of those oximeters. I'd like to have it just for my own benefit. I'm learning by reading this board, that you almost have to be your own doctor in most cases.
I am having trouble with the sleepyhead software. Both of my Macs have one operating system older than the software will support. I will see about upgrading. If not, is there an earlier version somewhere?
There is also an app for my iphone/ipad that phillips puts out that shows some data. Not sure how good it is.

Check this link and see if one of the older Mac versions will work with your older Mac.
I don't see the older Mac versions from last year anywhere. If you had Windows I could help as I have one saved that is no longer available.

It might help if they knew exactly what OS version you have on your Mac
You still have some data available on the LCD screen...do you know about it.
If worse comes to worse...I know of a way that you can send me the contents of your SD card and I can be sweet talked into running a report for you and sending it back to you via email.

The imac I'd like to use is 10.5.8
My macbook is much newer, but I keep it in my backpack, not out. If need be, I can take it out and use it. It is 10.6.x
You are so nice to offer to help with the data. If we dont get something going here, we can sure do it that way if you really don't mind.
I do know that my screen on the machine will show data. I am just not sure what it means.
it says 30 day AHI is 2.1 average
7 day is 1.2
1 day is 0.7
periodic breathing and large leak are both 0%
Did I read that the AHI number needs to be under five?

buy a recording oximeter, I prefer the cms50i, because it's wrist mounted, and records multiple sessions along with the date and times of them... around 100$ish from aliexpress.com if you want something faster, you can get the same thing from amazon, or other places, but for more $$$.

Then we can tell you how far up the OS X chain you can upgrade to, which is very simple to do on a Mac. Unlike Windows, it is easier to upgrade a Mac's operating system than to hunt for older software to suit your existing OS version. And it may cost you nothing to upgrade.

I had this reply composed yesterday when the forum gremlins hit so saved it.
Periodic breathing is a waxing and waning breathing pattern that sometimes can point us to a problem if someone has a lot of centrals....0% is what we like...so that's great but a little PB isn't cause for alarm as we can all do some every now and then. So don't freak if on occasion you see a little % there for the daily number. And I already know you didn't have enough centrals to be a problem because your AHI isn't high enough to be a problem even if all your events were centrals.
Large leak...this is percent of time in large leak...so you had 0 time spent in large leak...that's great too.
When you have time read my SleepyHead tutorial as it explains these values in more detail and how your machine reports leaks. The link to the tutorial is in my signature and the tutorial is also at the top of the announcements section on the forum main page.
Yes...I would be happy to run you a software report if we end up needing to do that. It involves a little bit of work both on your end and my end but I would be happy to do it.
I might even throw in an Encore Report for you just for grins so you can see what they look like.
Visually they are much different than SleepyHead but the data reported is the same and you can't use Encore on a Mac unless you put Windows on a Mac.
