Re: SleeyHead? Dreammapper?
Okay, I read the sleep study and have a better feeling for what happened.
Your sleep apnea is worse in REM (lots worse) and I am quite familiar with that side of things myself (my OSA is 5 times worse in REM than in non REM). Makes it harder to do a good titration because it's longer to get to REM and we don't stay in REM long enough to get a good idea what pressures might be needed.
Then you had a split study which means even less time for the titration part of the study. Simply not enough time and REM sleep to get a good handle on the pressures needed.
You titration report showed a few centrals...slightly up from the diagnostic study but not a horribly alarming number. I was worried that the cpap pressure might be causing the centrals flagged by your machine. It might still be a consideration but I am leaning towards the centrals simply being a by product of poor sleep in general due to the OSA stuff causing arousals. Meaning you sort of woke up or had an arousal (you may or may not even remember the arousal) and the centrals flagged are awake/semi awake breathing which is a by product of the arousal itself.
It's what I would call Sleep/Wake/Junk (SWJ) flagged events or post arousal SWJ.
My thoughts at this point....fix what we know needs fixing that we can fix with your machine at this time and once those things are fixed we sit back and look at the centrals and see if they are still numerous enough to be a concern. We can't fix the centrals with your machine UNLESS the centrals are indeed post arousal centrals/SWJ and if we better fix the OSA stuff the centrals will reduce as a secondary reaction.
I have seen it happen...fix the OSA stuff better and the centrals go away.
It is of course also possible that your centrals are from cpap pressure itself and if that is the case we get to have a different discussion about your equipment needs.
But lets start with simple and easy first because sometimes it works out just fine and that's what you have available right now anyway.
So I would suggest switching over to auto/apap mode and start with these settings.
Minimum 9
Maximum 20
AFlex or CFlex per whatever is comfortable to you or even off if you like it better.
Humidity per whatever is comfortable.
I am going to cross my fingers really tight and hope that if the machine can better prevent the obstructive stuff (OAs and hyponeas) that the centrals won't increase with the increase in pressure that I know we are going to see happen with the machine being able to go higher than 9 cm.
It's quite common for pressure needs to be markedly different during REM...so I fully expect the machine to want to increase the pressure significantly.
We may need to even give your machine a better head start than the 9 cm we are going to start with...but we have to start somewhere and 9 is as good as any. For myself...in REM I have seen my pressures increase 8 cm more than what I started with. Doesn't happen all the time but enough of the time that I recognize the need often.
If at any time the central numbers go horribly higher than what you are seeing now we abort the experiment and back up and have a different discussion.

Re: SleeyHead? Dreammapper?
Thus far, from what I have read:
1) You have a fully data capable APAP (DreamStation)
2) You have an SD card reader attached to your computer (Mac or Windows is unknown)
3) You now have downloaded SleepyHead and installed it.
The facts I have not been able to determine are 'how long have you been using the machine and how many nights data has been recorded on the card?' and 'have you erased any data from the machine or card?'
I don't know how often data gets written by DreamStation before being overwritten, but as I recall it used to be about a week or so for RemStar or System One. If you are lucky, your card has all the data since day one on the card and when you import it into SH, you will have a contiguous set of reports from which you can build a cohesive view of the machine's behavior and your breathing pattern. Developing a rhythm to importation is critical to keeping your SH data manageable.
At the risk of confusing the issues, in addition to Pugsy's knowledgeable tutelage I might suggest a series of videos on youtube produced by LankyLefty.
1) https://www.youtube.com/watch?v=5jCNeivvrjU (this one relates to Mac but once installed, it contains information that is useful for either platform)
2) https://www.youtube.com/watch?v=AxQ4a94DB68
3) https://www.youtube.com/watch?v=Z8egUCl2yJI (scoring severe)
4) https://www.youtube.com/watch?v=LT2Nhbspxkw (scoring normal)
Some like his presentation style, others can't stand it - in any regard, he seems to know whereof he speaks.

Re: SleeyHead? Dreammapper?
The week thing was the ResMed S9 machines only.....the new AirSense 10 models...won't over write for a year if even then.JimW159 wrote:I don't know how often data gets written by DreamStation before being overwritten, but as I recall it used to be about a week or so for RemStar or System One.
Respironics System One and the DreamStation models...it's well over a year most likely unless a victim of the gazillion session bug.
With the Respironics System One or DreamStation there is considerable data stored on its internal memory for at least 30 days...so even if the machine didn't have a SD card in place there is considerable detailed data available and enough to get started in the evaluation process easily.
Flow rate data graphs...not stored on the machine but the other graphs are.
Unlike the ResMed machines where none of the detailed graphs are stored on the machine.

Re: SleeyHead? Dreammapper?
Hi guys. Sorry. I just finished reading everything on this thread. I didn't realize there was so much. I'm still trying to get my way around tapatalk and sometimes hard to find my threads again.
I've been using CPAP for 4 nights now. I was only just diagnosed on 3/11 this year (Aussie date format sorry) . I only just got the machine on 19/12. It took me that long to get the results of the study then see the sleep specialist. Diagnosis done through the sleep study above. I'm very reluctant to adjust my machine till I have been approved.
The hospital can "permanently" lend the machine to me as long as I "qualify" which I have to do a 6 night trial to qualify. I don't know what I have to show to qualify as that was never extrapolated to me. I hope it just means I have to show through machine data that I have been compliant and that I actually need the machine. If so I should qualify with both eyes shut.
I am extremely disappointed that the specialist just read the report and probably only read the one paragraph that said 9cm was adequate.
I totally agree with Pugsy that its not as easy as a one pressure setting fix. If specialist had have read a bit further he would have read that titration was not successful which should have rang alarm bells to put me on auto and work towards the right fix. Thank God for auto machines these days.
Since my next appointment with provider is 5/1/18 and I don't even have an appointment for specialist yet I just have to sit tight till I see someone again. I just love our public health system here. Give as little as possible and hope it works. If it doesn't. Too bad too sad basically.
I've read some of the data on SleepyHead and I can see there's data on the card from February this year. Is there any way of clearing that data without deleting or affecting my 4 days data?

Fortunately you have learned enough which will help you obtain proper treatment much much sooner. Some people go years before they learn that their therapy sucks. Others never learn.

Re: SleeyHead? Dreammapper?
There is no way to permanently remove prior person's use data from the SD card without removing your data but there is a way we can remove it from the SleepyHead report. Couple of ways in fact. How much data are we talking about?
If it is just one or two nights then you can just manually go to those dates and turn the sessions off or you can use the purge thing (which sometimes is a bit buggy) but will let you do large blocks of removal in one step.
To do it with the sessions just navigate to the dates involved and look on the left side of the daily detailed graphs page where the statistics columns are and scroll down until you see the sessions shown and there will be a green bar by the sessions. Just click your mouse on the green bar and turn it red and that will turn off the data reporting for that session and remove the data from your results.
I just looked at my SleepyHead and I can't find a purge that will let you do selected dates. Darn, I thought that was available but I can't find it right now.
So until I can find it just do the turning off of the sessions thing.
We could remove it from the SD card but the machine has it stored in its internal memory so will just put it right back on the SD card so that won't work.
Now there is a way to remove it from the machine itself but it will also remove your data and of course that is not wanted at this time.
If we had known it could have been done prior to your using the machine or the people who loaned you the machine could have done it before you were given the machine.
I don't know how things work in Australia with cpap and DMEs (people who supply the machine) but in the US a DME can't change a pressure setting without a doctor's okay. There's no rules about the patient doing it though despite the squawking the DMEs might put forth when we do.
Your choice...you can change it to auto mode now or wait and see if they will do it.

Re: SleeyHead? Dreammapper?
Thanks Pugsy. I want to at least speak to them before I do. I don't have any info about the trial whatsoever except that I have to pass before they'll let me keep it.
Anyway. Last night's results below. 5th night and worse results yet. Went to sleep about 2 am and woke about 8 am. Had a friend over for dinner and wrapping presents after that.
I was pretty zonked so probably had more rem sleep than normal. Feel like a train wreck today.
Went to church with my wife and two daughters this morning and virtually slept through the sermon. 1 pm now and feel like I would sleep for a week if I went to bed now.
Have a great Christmas all and don't let it wear you down. It is a festive season after all.

Please review this thread for the format we like to see the daily detailed graphs.
viewtopic/t103468/Need-help-with-screen-shots.html
You need more pressure or a change to auto mode which you don't want to do until you see your provider again. My original advice still stands.
Fix the known obstructive stuff first and then see if the centrals still remain in numbers that are a potential problem.
The bulk of your AHI is obstructive in nature and not central.
Until you use more pressure or let the machine use more pressure by going to auto mode your AHI is going to be too high.
