AHI recently increased
AHI recently increased
Equipment:
Respironics System One with humidifier
Wisp Nasal Mask + Chin Strap
Preset CPAP pressure 13cmH2O
Length of use 2.5 years
Average AHI number 2-7
Two weeks ago I had two cardiac events (heart attacks) and am now on Beta Blockers and diuretics while in the recovery stage before I can start Rehab.
When I checked the onboard Data information on my CPAP machine it indicates my AHI has increased to the 16-22 range. Periodic breathing has increased also
to sometimes greater than 45%, Large leak is zero.
My question is why the sudden increase in AHI?
My pulmonologist doesn't believe in AUTOPAP insists the 13 cmH2O is correct.
He will only prescribe melatonin as sleep aid now (tried it and didn't the side effects). Prior to that he only recommended Lunesta which has worked for me in the past.

Re: AHI recently increased
Two weeks ago 2 heart attacks...... when that happened to me it took some time for my body to settle back down to a reasonable pace. You have had 2 heart attacks, and are taking some pretty heavy heart drugs...give it time .. and don't add another drug to the mix, just try and relax and let your body heal. Once things settle down with your heart and whatever else it has jogged you will SLOWLY get back to your normal self and your ahi will go down.
Good luck with your recovery.

Good luck with your recovery.

Re: AHI recently increased
Beta blockers are considered to be counter productive to using Cpap by many.

Interesting, how so?Julie wrote:Beta blockers are considered to be counter productive to using Cpap by many.
Beta blockers post MI are used to decrease the workload on the heart while it heals.
With OSA the CPAP still has to be used.
Thanks for the input.

Which specific model do you have? Is it a data-capable machine? It appears to be from the numbers you posted.aghauler wrote:Equipment:

Are you using software (like Sleepyhead or Encore) to SEE what's going on at night? If not, why not? You NEED to. It would help answer your questions.
And, due to your heart conditions, your pulmonologist has it exactly right.

Undetermined at this point, may only be during the recovery - healing phase.Julie wrote:Does your MD want you to be on BBs long term?

This CPAP does have a data card. Didn't know anything about the available software, and had wondered about it's availability. Also thought if I printed the data on the data card it would erase the info so it wouldn't be available to anyone else.Wulfman... wrote:Which specific model do you have? Is it a data-capable machine? Pulmonologist and staff are very controlling and not very empathetic to my situation, this was before any heart issues showed up. The "too bad you have to drive 3 hours to see us, don't try any other mask, I'm holier than thou staff attitude" has turned me off to them and I've currently turned my CPAP issues over to my Primary Care Provider. The local vendors don't like dealing with this particular Sleep Lab group due to their dictatorial attitude.
Also I should add I've been a Respiratory Therapist for 45 years, so know "something" about BiPaPs used clinically, but not too much regarding CPAP machines as that is the Night Shift realm. I've setup and managed numerous BiPAPs on Day shift, never have setup a CPAP on Day shift though.

WHICH SPECIFIC model of machine do you have?
There are a few low-end machines that don't collect data but still have data cards in them.
You may have to peek at the bottom of your machine to see the model number, but if it's "450" or higher, it's fully data-capable. If it's lower than that, it's not. Model names that are data capable are "Pro" or "Auto".
See if this link will help.

This site was developed by a member here, Krelvin, and has a lot of info on how to download and use Sleepyhead. Be sure and click on all the links to see how to organize your charts and post, if you want, for further help. https://sleep.tnet.com/resources/sleepyhead
Also if you tell us the REF# on the bottom of your blower, be sure and remove you humidifier before turning upside down, we can point you to where you can get the clinical manual to see how to change settings if you want.
This link may help you identify your machine. https://sleep.tnet.com/cmd
Like Wulfman said some do not record detailed data. So we need the specific model to give better help.

Re: AHI recently increased
Good! That means it is fully data capable and you can download the SD card to sleepyhead to see what is going on. Sleepyhead does not erase your card but you want to be sure and slide the card lock to lock when you put it in your computer so the computer doesn't write any unwanted files, and the slide it back to write when you put it back in your machine.
If you have any questions about downloading Sleepyhead ask and someone will help. Use my links above.
