Reras on Respironics-Bipap ASV

Post by Boop » Wed Mar 12, 2014 8:56 am

I was reading somewhere on the forum that Respironics flag reras. I have a ASV 960 by respironics. I use Encore pro 2 for data. I don't see any data for Reras. Can anyone help me?

My records show the Encore Pro does track and report RERA's but not on the 960 machine.
They are reported on my previous 750 machine but not on the 960.

Too bad since it would be interesting data.
If the 960 collects the data, Encore Pro doesn't report it.

Perhaps SleepyHead in some future version will resolve the issue.

I use the 960 machine with Encore. I have never seen anything about reras in the reports.

I suspect that the model 960 machine simply doesn't flag RERAs and that is why it is absent on Encore Pro reports.
If the selection is absent from the events graphs themselves...it isn't collected.
http://www.healthcare.philips.com/pwc_h ... _Guide.pdf
This pertains to the ASV machine reports...I don't see a listing for RERAs in the events graph.
I am betting that the machine doesn't flag RERAs and instead does the patient triggered breathing stuff as I imagine that is much more important than RERAs

Where SleepyHead won't show so and so event in the events graph if none occurred...Encore will show the event category even if nothing occurred in that category. If it isn't on the detailed events graph....the machine and/or Encore doesn't flag it.
I don't think SleepyHead (even if it would work with the 960s) would make it show up.

I don't think the BiPAP ASV flags RERAS but the other Respironics machines will. I have the 950 model of the same machine and have never seen RERAs in Encore either. Sleepyhead will always show the index as 0.00. Both Encore and SH can only report what the machine collects, so if neither is showing RERAs, it's pretty safe to assume the ASV units aren't tracking them.

Someone will probably chime in with a more official answer, but my guess is that because the ASV supports your breathing on a breath-by-breath basis, it might assume that RERAs won't occur and therefore don't need to be tracked/reported (i.e. your flow rate would have to be completely unresponsive to ASV-delivered pressure support in order to match the pattern for flagging a RERA.) In other words, the ASV can alter your flow in a way that prevents the machine from finding a flow pattern that matches Respironics' definition of RERA.

It doesn't mean you aren't waking up due to respiratory events, even if nothing is flagged. The flow rate just doesn't match the criteria to be considered an apnea, hypopnea, flow limit or RERA. At least, that's what I tell myself after a bad night with good numbers

Encore Pro (earlier version) doesn't report minute vent, tidal volume, and some other parameters that SleepyHead does report for the 750.
Encore Pro (current version) reports these parameters for the 960.
So it is not clear to me that the RERA data is not collected and so not reported or is collected and not reported.
Encore Pro does other weird things in reporting and this could be just another of them.

It really bugs me that Encore Pro chooses to truncate the waveform data to about the most recent 8 hours of the last night's data.

Thx everyone. I guess for this machines it's not that important. I am still not sleeping long enough yet and I thought maybe it might have something to do with Reras. Just trying to touch all bases. I am trying the Cognitive Sleep Therapy that the doctor advised. It's pretty hard. Thx again

I learn something new every day! Didn't know that the earlier Encore was different. Hopefully those with the older version have already caught on and been able to update so they can see everything.

SH will show a RERA number if I turn the BPM off (currently using it in CPAP mode per doctor recommendation.) RERA index will usually be something like 3000, which I chalk up to an SH error. Even on these nights, Encore doesn't report RERA, so that's another reason why I assume it doesn't collect that data. Haven't noticed too many weird reportings in Encore Pro yet. Anything in particular that seems to be "off" consistently?

I haven't noticed weird numbers but in the way the data is handled.
1) If you don't upload every day, the in between days waveform data is not uploaded although the machine captures it on the card. If you have a version of SleepyHead that is compatible with your machine, it does get uploaded.
2) If you nap early in the day and upload before that night, you can lose that night's data.
3) As I mentioned before, if you sleep much more than eight hours, the only the last eight hours of waveform data is captured the beginning of the session gets truncated. This applies to only the wave data and not the detailed/summary data. But, if you have a long break at night fighting insomnia AND you turn your machine off for that period AND the total hours excluding the insomnia period are somewhat less than eight hours, then you get all the wave data.
4) If, on the other hand, you get too curious during the insomnia period and download then, the rest of the night may get lost completely.

Weird!
