Problems after getting a new machine...

So, I've been using a F&P HC254 for about 5 years now, and its been pretty good. I was concerned that it was getting old enough that I would be without should something happen, I got a new PR System One Auto CPAP (w/Humidifier). I've been using it about three weeks, and while my AHI has not increased measurably, but I don't feel like I'm sleeping very well. I'm waking up quite sweaty, and some symptoms I had before going on to CPAP have begun to return. For the first time last night, I went back to the HC254 after three weeks and got a decent night's sleep. Has anyone had this same problem with a new machine? I even turned off the A-Flex because it caused my full face mask to shift weirdly on my face.

Different machines...different algorithms. Are you using the exact same pressure settings as with the F&P?
Have you looked at your software reports to see if there is anything going on in the reports?
You might need a slight pressure adjustment.

I would try to duplicate the old machines settings as closely as possible on the new machine. What are your AHI readings on the new machine? Do you have centrals showing on the reports now?

Of course the first thing to look at: how is your leak line in the graph data? That may go a long way to solving some issues. Where there is large leak, there is no therapy.

Another possibility:
For some of us, the algorithm used in the flow generator makes a difference. I can tell a huge difference between the Respironics algorithm and ResMed's. They only way I could use my M-Series Pro was to turn off C-Flex, and even then the flow generator seemed to be fighting me. The S9 seems to suit me better. It's not that one machine is better than another. Rather, some machines are more suitable for some people.

If you do really well on F&P, perhaps you should consider getting a new F&P instead. Alternatively, if you are looking for something with lots more data, the ResMed S9 may be a better fit for you than the PRS1.

That was the one thing I had yet to do (and I realised it after my post yesterday). It started out as 4cm-20cm auto, I bumped it up to 8cm on the low end, but I know my F&P is 10cm on the low end. I'll bump it up and give it a shot tonight.

Any where between 2.5 and 3.5.

That's a good question, I'm unfamilar with the data output of the PRS One. I do see some Clear Airway Apneas but normally less than 3. Had one night of 7 and another 10, but I believe that was the week I was just getting hammered when I was oncall and wasn't really sleeping much.

Quick followup:

After bumping up my pressure to 10-20cm, things got much better.

My AHI is been anywhere in between 1 and 2.5, mostly betwen 1-2.

I'm seeing the occasional Central, maybe 1-2 a night now.

The % Large Leak has been less than 5% generally.

It sounds like your machine is serving you well now. It also sounds like your ability to detect ineffective treatment is good. Enjoy your machine.
