Headache diary - great template

I am posting the headache diary that my sleep doctor gave me. If you happen to live in the Seattle area, I highly recommend him.

The front side has room for 2 months at a glance - easy to see patterns. You mark the squares for the appropriate day and time with the level of pain the headache is. Down below, you list the pain reliever options on the left, and you mark the box with how well it worked. There is also a place to mark which trigger you think it was. The second page has a list of possible triggers.

If you are serious about treatment for headaches, it may help to go to your appointment with a headache diary that shows the quantity and severity of your headaches in an easy to view chart like this.

I had to reduce the graph down to 50% for my pc to be able to see all the verbage. Therefore, please forgive the spelling since I can't really tell if I'm typing correctly. I very seldom get headaches. However, my daughter gets some nasty ones. I'd like to send this to her if it's okay with you. Can you let me know how I can send her an email with all this information on it?
I'd really appreciate it very much.

You should be able to right click on it and then save it to your computer. Then attach it to an email. I left it large so that it could be printed as a full size sheet of paper.

If you can't get that to work, send me your email address in a pm, and I will email them to you.
