Hello, I'm sure these topics have been gone over. I have been reading a lot of the posts. Here is what I was diagnosed with:
I only made it 1.5 hours into my sleep study. Which quickly became treatment. The results Were 123.5 AHI. Basically explained to me as breathing for 10 Seconds, not breathing for 20 seconds. Repeat. I have 100% blockage and have been issued the following:
a pressure prescription of 18 over 14 (Bipap machine) I got a Resmed S9 with humidifier and I received 2 masks. 1. a Respironics comfort gel full face. And also a ResMed Quattro FX.
Night one started out a bit rough. I apparently slid down my pillow and broke the seal on my mask. I did this for an hour. I woke up 1 hour later. Tried to adjust the mask. Finally got up, looked in a mirror and adjusted the mask. I think there was still some leakage thru the night. But I made it 7.2 hours with it on. And I feel awesome, no headaches, Blood pressure normal! I can't wait till tonight. I do have some questions though:
The only thing that I saw that my S9 would give me for info was time and the seal for the mask is a smiley face. I would like a little more detail on how to get more info out of my machine. The AHI was 32.00 Which is high and probably due to the mask not having a good seal that first hour or so.
I read about a leakage rating that people are talking about. I would love to know where that shows on my S9. I fell great so far and want to know that I am getting the best results I can!!
Any Help and info would be great!! And I apologize in advance for the redundant questions.

Download and install sleepyhead. The memory card has the data you seek. The machine doesn't give you the information. The modem in the back sends it to the doctor and DME.
Pull the memory card out and insert it into computer via slot or adapter. The software gives you the option of uploading data.
Did you have memory and or cognitive problems? I am just starting to see a difference after three months of CPAP. I seriously doubt we ever get our brain back. The damage due to low oxygen is irreversible. I am doing all my math in my head and constantly challenging myself.

The AHI is the main number that the sleep docs look at when deciding whether or not your therapy is considered effective: They want that AHI number to be consistently less than 5. (Note that an AHI < 5 on a diagnostic sleep study is considered to be "normal"; hence effective therapy means that with the machine you have no more apneas than a normal person has on a per hour basis.)
Now let's suppose your overnight (one day numbers) one morning are:
First the AHI, AI, CAI, OAI, and HI are all indices. They indicate how many events of the given type you had per hour on average during the time the machine was on:
Now let's look at what the rest of the numbers you see on your S9's LCD or in ResScan or SleepyHead. (These are software packages that you can download for reading the data on your S9's SD card. ) I'll start with some really important key definitions so you know what these numbers mean. It's easiest to start with some examples. In these examples, I'll continue to suppose that you slept with the S9 running for 7.5 hours and we're looking at the overnight data in ResScan or SleepyHead or off the LCD for the numbers it shows. Now let's look an example of some data: Let's suppose one night you're leak data looks like this
The 95% leak rate = 7.2 L/min means that for 95% of the time the S9 was on, your leak rate was LESS THAN or EQUAL TO 7.2 L/min. And so for 5% of the night, your leak rate was GREATER THAN or EQUAL TO 7.2 L/min. Now it's important to realize that 5% of one hour is equal to three minutes. Since you slept for 7.5 hours, that means your leak rate was GREATER THAN or EQUAL TO 7.2 L/min for a grand total of (3 minutes)*(7.5 hours)=22.5 minutes. That 22.5 minutes of "high leak rates" might be in one giant half hour leak or it could be in several smaller leaks---the only way to know is that to look at the detailed graphs in ResScan or SleepyHead.
The max leak rate=15L/min indicates that at some point when the S9 was sampling leak data, it detected a leak that was as large as 15L/min. It may have been a very short lived thing---when you broke the seal to scratch your nose for example. Or it could be that on one or two of the big leaks that you had, the leak pretty quickly went from in the neighborhood of 2.5 L/min to 15 L/min and stayed there. But you know for sure that you could not have leaked at a rate of 15L/min for more than about 22 minutes because of the 95% leak rate number and the time the machine was on.
Interpreting the leak rate data is also important:
First, on the Short Sleep Quality menu, if you see a Red Frowny face, that indicates your 70% leak rate is AT or ABOVE 24 L/min and Resmed's litertature indicates that you are leaking AT or ABOVE a rate of 24 L/min for 30% of the night. And at that point, the ResMed engineers believe that your therapy can and probably is being compromised. Your posted leak number is significantly below this rate, hence the S9 does not believe your leaks are sufficiently large to compromise your therapy.
Many people take the 95% Leak Rate to be the key stat Leak data. But it is worth looking at the median leak rate as well, in my opinion. The reason I look at both is that the median leak rate tells me whether I was leaking off and one all night long or not, where the 95% leak rate tells me if my worst leaks were bad ones or not. To illustrate what I mean, let's look at two examples:
Person A sleeps for 7.5 hours and is leaking off and on all night at a rate of 5.5 L/M and for about 25 minutes during the night, the leak rate actually is between 7 and 10 L/min. So A's leak numbers look like this:
median=5.5, 95%=7.0, and max=10.0
Now suppose Person B sleeps for 7.5 hours and has NO leaks at all for most of the night, but has ONE 25 minute leak where the leak rates are from 8 to 10 L/min during this leak. B's leak numbers look like this:
median=0.0, 95%=8.0, and max=10.0
Neither person's leak rates are large enough to trigger the Red Frowny face in the short Sleep Quality menu on the LCD. But Person A is likely to be having more significant problems with comfort due to leaking all night long.
Obviously, the closer to 0 your leak rate data is, the better obviously. But most people are not going to have the 95% Leak Rate equal 0.0 night after night. So what's a good upper tolerance on that 95% Leak Rate number?
Well, the information from Resmed indicates that leaks above 24 L/min (the RED LINE in ResScan's detailed graphs) are enough to cause problems with the algorithms the S9 uses in order to detect and respond to apneas, hypopneas, flow limitations, and snores. So in any case you really want your 95% leak rate under 24 L/min to insure that you're getting some kind of meaningful therapy. But comfort is as a much of an issue as therapy is in my opinion: Nasty leaks cause most people real discomfort and they frequently cause people to wake up multiple times during the night to fiddle with the mask even when the leak is well below 24 L/min. For me, I don't worry at all if my 95% leak rate is below 10-15 L/min unless I had to fiddle with my mask more than once in the night. And if I wake up multiple times needing to fiddle with my mask because of leaks, then the leak rate IS a problem regardless of how low the 95% leak rate is.

- The S9 will score an central apnea when your airflow is reduced by at least 80% from baseline (i.e. you're not breathing) lasting at least 10 secondsAND the S9 has determined that there is appears to be no obstruction in the airway
- (2.3 events per hour)x(7.5 hours) = 17.25, which equals about 18 events that night because the S9 truncates rather than rounds
- (1.4 apneas per hour)x(7.5 hours) = 10.5, which equals about 11 apneas that night
- (0.3 central apneas per hour)x(7.5 hours) = 2.25, which equals about 3 central apnea that night

Well! night 2 with the machine. Used the ResMed Quattro FX last night. Loved it!! very comfortable! Much better than the Respironics comfort Gel mask. Also less noticeable leaks.
Downloaded sleepyhead. This is what I was looking for!
So now I have an average of 6.59 for leaks and my first night AHI was 30.47. And last night was 22.79.
Question: What are realistic good numbers? I would obviously like the AHI to be 0. Is this possible? I think going from an AHI of 123.5 to 22.79 is drastic. But what should I be striving for?

AHI of 0.0 is not realistic though some people do see it. Don't go chasing that number though. It will drive you nuts. Generally AHI of less than 5 is considered acceptable.
Could you please add your equipment to your profile as I have my equipment? It helps us help you better because we can see at a glance what you are using.
Your AHI of 22.79.....what was the breakdown into event categories?
The Central Index? The Obstructive Apnea Index? The Hyponea Index?

No centrals.
Obstructive events split almost 50/50 with obstructive apneas and hyponeas. AHI 22.something
IPAP 18 and EPAP 14.
If your AHI doesn't continue to drop you may want to talk to your doctor about it.
We like to see less than 5 AHI. These pressures may not be optimal but too soon to tell for sure.
Are you waking up often for any reason?
While 22 AHI is a huge improvement from your precpap nights....I would want it much lower if it were me. It may still drop on its own but I don't know if it can drop that much. Hence the watch it and talk to doctor about it caution.
Did your sleep studies happen to mention more events when supine sleeping or REM stage sleep? Either one is common and might explain the higher AHI if you happen to say be worse in REM sleep and you didn't get much REM during the titration study and now you are getting normal REM. Just a thought.

Last night I woke up 3 times. As for the REM sleep. In the sleep study I was getting 0 REM sleep. Then with the mask at the final settings 18 over 14 the Last 1.5 hours I was in REM. I think I am just waking up because I am not used to the mask yet. My wife says my snoring is pretty much gone. I have my first follow up appointment with my doctor on Wednesday.
I think maybe the Technician that I had during the sleep study was concerned because the office called me 6 hours after the study was over and told me that my prescription was waiting at the Medical Supply provider. So I went there later in the day and received all the equipment. Which I thought was a little hasty considering they scheduled my Follow up appointment with the Doctor at the same time they scheduled my sleep study. I would have thought they would have given the info to the Dr. from the study and then the Dr would give me the news on the follow up. But as it is now, I will have already been on the Machine for 6 days when I see him.

Not unusual to get the machine before seeing the doctor for followup. You are lucky some people have to wait several weeks to get a machine and you obviously need it pretty badly.
When you go to the doctor be sure and request copies of both your sleep studies.
Lack of REM during sleep study (most likely because the events kept waking you up and preventing normal sleep stages) is fairly common. I had 6 minutes of REM during my titration study and my OSA is hugely worse in REM sleep...so they didn't quite give me optimal pressure during the titration study.
Talk to your doctor about your AHI. See how he wants to address it. Might take some copies of your reports but he may also have ResScan so he can generate his own....providing he looks at that stuff. Some doctors don't and will go entirely on how you feel.

I really only have one question. When I get up to use the restroom. Do I turn off my machine? or just let the hose run. I was just letting the hose run but then i though that could mess up the figures.

Letting the machine run may give you a falsely elevated leak numbers but otherwise there is no critical need to turn the machine off if you don't want to.
If you can get your AHI down below 5 the need to urinate during the night should also decrease.
Don't know your gender but if male and age some of the need to urinate may be from prostate.
Nocturia (need to urinate during the night) is a common symptom of sleep apnea. With optimal therapy the need to urinate will often go away if it was related totally to sleep apnea and the production of the stress hormone that puts the kidneys into overdrive.
If you can get your AHI down below 5 the need to urinate during the night should also decrease.
Don't know your gender but if male and age some of the need to urinate may be from prostate.

yes, I am Male, 40
I think I will turn it off then. I am really wanting the leak rate to be low. I guess maybe thinking that will help AHI. If not, I am seeing the Dr on Wednesday. So hopefully it will get fine tuned. I have been feeling great already! No headache in the morning, blood pressure is great, no fog, no nodding off while sitting down to watch tv. If this gets down to under 5 AHI, I can't imagine what it will do!!
I have also found with the Full mask. I actually have really been trying to breath thru my nose. And last night I think for part of the night I actually was. And it was great! Im going to keep working on that. I am so much more relaxed when I can actually breathe thru my nose.
