I have been following my data on AutoScan and find that I have less than 2 AIs during the night but close to 11 HI events. I have the autoset at 10 to 18. My titrated value is 12. The median shows up as 14 and 95 percentile at 17.4. I do not understand the HI being so high. What causes this?
Would the next step be to have a shorter range, say 11 to 16 or try the straight titrated value of 12. My doctor doesn't believe in apaps and says 12 is the answer but I felt terrible on that and now I am feeling not so good on this. I have been sleeping long periods again which is something new. I was doing 8 hours at 4 to 20 on the two-week apap study. Now I am exhausted. I slept 12 hours last night and am having difficulty getting up to go to work.
I have a Resmed Vantage. I have very little leak problem with my nasal pillows. Swift, Breeze, or Nasaire. I guess I should have known there would be days like this....I sure would value some opinions.

I've been seeing something similar; as my AI goes down the HI and FLI go up. I think of this as a good thing though; these events would have been Apneas, however, because of the pressure were converted to Hypopneas or Flow Limitations, which cause fewer problems. Usually, the oximeter doesn't even register much in the way of desaturation for hypopneas.
I'm still not titrated very well myself, but what I have found helpful is to make small adjustments and try them out for a few days to see what difference they make. Sometimes it's been real obvious that the adjustment is going in the wrong direction, but usually it takes longer to decide.
Hope this helps a little.

Bill
alphabettie,
Are you experiencing dry mouth upon waking? If so, this would indicate you're leaking pressure out your mouth and might also account for your pressure increasing. Mouth leaking would necessitate using a chinstrap or taping your mouth shut.
It also sounds like 14 MAY be where you need to be pressure-wise (if you're NOT having a mouth leaking problem).
How long have you been on CPAP? It's also typical to go through a period of time before the therapy starts to give you the feeling of being "refreshed".
Best wishes,
Den

I agree with Den about the mouth leaks, but wonder at the pressure and the way you feel. Is your HI now higher than before?
What was the 95th percnetile pressure in the two weeks you had the machine on 4-20? When you say "my titrated pressure" do you mean the pressure titrated in a sleep study, or based on those two weeks?
Did you feel well during those two weeks?
What were your indices then?
Did you originally have a high HI in your psg?

I have no one to tell me if I have mouth leaks or snoring. I use a red chinstrap and last night I taped my mouth shut. I thought I was a nose breather. Only twice since Dec 9 have I had a dry throat and mouth. I do not know what happened over the two week test in that all the doctor did was tell DME to keep it at 12. That is where he had me on the first day: December 9, 2005. During the two weeks, though, I felt better than I did on straight 12. I did have a rather high HI but do not remember what he told me. I have no copy of it. Thank you all for your response. I think I need to request some information.I agree with Den about the mouth leaks, but wonder at the pressure and the way you feel. Hello just like you I have the S8 Vantage, I use the full face mask. I am getting readings similar to yours my average pressure is 12.5 my AI is 0.9 and my HI is 11.1. My pressure is set from 9-20 I was told by my Doctor after using one of their autopaps for 3 night that my pressure should be 12. I wonder if it something to do with the machine or if we are really having HI of that high? I guess only time and another sleep study will tell. But until that happens I would like to hear more input on this, I am very curious.

Something was happening
Soon after I posted the initial post, I started coming down with a virus. Still recovering. I am sure that things will improve soon. Sure hope so. I did the best I could with the Breeze but my nose is so red and sore now. Plus I slept like all the time. Maybe I will get a grip on this in the future.
