Ahhh, another day, another utterly terrorizing / utterly hilarious trip to the cardiac wing of the ER.

Admitted overnight, I realized I didn't have my S9 AutoPap.. Nurse says: "No problemo."

Forty-five minutes later she wheels in this thing. On the spot, I nick-named it "Godzilla".

At around 2 a.m., someone else came in, I think it was the night-shift RT, and she's not super familiar with the V60.

I tell her let's just do CPAP at 10.4, best she can do is 10.0. .Why?..Because it titrates in increments of whole centimeters: so it's either 10 cm H2O, or 11 cm H2O — nothing in between.



THE MASK:

She unveils a no-name throw-away nasal mask. Must've cost 50 cents to manufacture.

A spider-web tangle of impossible-to-adjust spaghetti-thin cloth straps. The mask itself boasts razor-sharp inner edges.

No this isn't it, but the above would be a distinct improvement.


The RT practically puts her foot on my chest, cinches this mask hard.

Like she's strapping an Apollo astronaut into his reclining space-chair.

She flips the V60's switch to "on", asks if I'm good.. I gasp "yes", and she says a cheery good night.

Fade to black.

I'll post the rest later, but for now, I can say this much: if it wasn't for my military training at CPAPtalk, I would've torn the mask off my face, and left that hospital DAMA.

Nurse Helga: No prob. We'll just put him on the OR schedule for a D&C. We'll see what comes back.

Nurse Nightingale: Oh BTW, you got a FOM posting out of 2 Bd-2 Bth. Did the attending get a diagnostic?

Nurse Helga: Yeah, classic DSM-IV 297.1, GD subtype. In reality, he's just a grunt CRT — and get this: not a single entry in the literature, not even a letter to the editor in Moldavia. . .

Nurse Nightingale: But he acts like a fully published MD?

Nurse Helga: Uh-huh. Don't you hate when their type don't understand how easy it is to fast -food consume the literature, vs actually having to put your a** on the line, and produce the literature?

Nurse Nightingale: Drives me crazy. Oh, btw, is he a hit 'n runner? Will we see him in this ward again?

Nurse Helga: Unlikely. His history shows him as a definite H&Rer: any sign of real contact in the trenches, and he's gone as fast as The Road Runner.

It's a ventilator, but with CPAP, APAP and BiPAP options.

However, when you're on a hospital bed, and it's towering above you, it looks like Gozilla.

Not at first.

But I hadn't slept in 48 hours, and knew it was important to go to sleep, on PAP.

I got up, began playing with the onscreen controls.

First at bat: a very annoying beep, beep, beep when there's sufficient leakage, or some other parameter is met. .I had to play around for a bit, going from screen to screen, until I figured how to silence that beep for all events.

Second at bat: At home, I use a ResMed S9 autopap, set at 10.4 to 15 (my settings, the nyc sleep doc is MIA, never hear from him, even if I call or email). .So I went to the AutoPAP settings (not labeled as AutoPAP, but something else), set the range from 10 to 15.

Popped on the mask, the V60 has an autostart when you start breathing, and the air began to flow in. .Way too hard, way too fast, felt like the flimsy mask was going to be blown right off my face. .I pulled the mask off, confused. . Then I realized this is a Respironics unit, not ResMed, and obviously uses Respironics' algorithm. .Last time I used a Respironics APAP, I didn't employ a ramp, and remember the titration going from zero to sixty, max set pressure, in a few seconds of starting. .Haven't seen others here complaining about that, so perhaps just two outliers in a row, who knows.

So, I went back to the screen, found the CPAP settings, figured straight 10.0 was close enough for disco.

THAT DAMN MASK

Rested gal has insisted: "The mask is the #1 key to being able to "do CPAP, and I have to agree.

But this Fiddy Cent mask was seemingly built to keep you from sleeping: razor sharp edges cut press against your skin, and you'd need to be a swami, training on a bed of nails, to fall asleep with this cheap toy. .Course, (not-an-MD, but-I-play-one-on-CPAP-BBs) Moe wouldn't know anything visceral about this stuff, he's never had to wrestle with a mask in his life, gaping hole in his Pre-Copernican POV.

I played with the tiny cloth straps, about 1/4 of an inch wide, two velcros on each strap, they get caught in the thing you thread them through constantly, so it's a real bitch trying to get the right length. .After, maybe 40 minutes of experimentation, air on, lying on my left side (as I always do, fall asleep and awake in that position), I finally got the straps on as good as I was going to get.

But it still hurt like hell, those damn sharp edges. .And it was leaking straight into my left eye, no reasonable strap setting seemed to fix that.

So I amble up to the nurse's station, ask for some Vaseline. .Get a whole bunch of vaseline "packets", like mustard and ketchup come at Mikey D's and other fine dining establishments. .Back into bed, rub the stuff thick all over the sharp edges. .Give it a try: much better, no sharp edges. .But it still leaks hard into my left eye. .So, I rip a tiny bit of tissue cloth off, ball it up to fit in the eye crook of the mask, cover it with vaseline. .Couple of tries, and bingo, no more eye leak.

A couple of deep slow breaths, mentally go to my "special place" (Winter Storm, Vostok Station, Antarctica) and fall deeply asleep.

When I awaken, I check my recently purchased CMS 50D oximeter (I calibrated it when I was in the ER, bored, and playing with their equipment), and while it's obviously no Masimo Rainbow SET Rad-xx, the 50D seems damn accurate for it's absurdly cheap price.

My SpO2 was 99!

Then I realized that the V60 has an oxygen feed, so . . .

Anywayz, all of the above would never have happened without my rather short stint at CPAPtalk, and the generous people who've actually helped me here.

Had I not gone dial wingin' at 2 a.m., I wouldn't have slept for at least eight hours (till shift change) after suffering a major cardiac event, and I had not enjoyed sleep for the previous 48 hours.

Or I would've slept with an untitrated AHI of 71ish.

Both scenarios a potential disaster.

Not a "bigger picture" kinda guy, I see.

Hey Mike, thanks for posting the pic -- that's the freaking scary thing I got stuck with following a trip to the ER where they wouldn't let me leave until I had 3 days of IV antibiotics. The durned thing is so huge I was terrified I might pull it over on myself if I turned over in bed. Me the confirmed pillow-mask user with severe claustrophobia & fear of suffocation ended up with a nasal mask the RT had to put on me with my eyes squeezed shut so I couldn't see the beast on my face...that was about 1AM with me telling myself not to open my eyes and not to panic. RT added O2, he said it'd help with my anxiety and the infection. OK, so it's lights out and I try to fall asleep...the mask leaks...hideously, into my eyes. Plus, it hisses when I breathe...now, that hissing is the thing my brain hears then wakes me up to close my mouth (pillow mask wearer remember) so I wake up, fiddle with the mask to reduce the hissing, settle in again and the hissing returns to its previous volume and I wakeup again, and again, and again. Finally, about 4:30AM, I'm so exhausted I fall asleep only to be awakened at 6AM for vitals check and blood draw to see if they can culture what's growing inside me...I tore the damned thing off my face, I freaked when I woke up in daylight with horribly bloodshot, dry eyes (nasal mask evil). Later that morning I call my mom and walk her thru the steps of disassembling my APAP and bringing it to me. I was never so glad to see a piece of equipment in my life! Finally after more tests, I get to sleep, slept thru several changes of IV and saline and I normally wake at the slightest sound or noise! Moral: don't go to the ER without your gear.

I sure hope all is well with you and full recovery is imminent!

Thanks, ems. .I do feel a lot better.

OT: Beth Israel has the potential to have the best ER in the city — their triage management still needs tweaking — but I made the right choice in going there.

My wife is still miffed that I didn't choose NYU, where she's a POM, but I've been to NYU's ER twice, and wasn't thrilled.
