I love Titrations
I love Titrations
Its the morning after my 2nd titration at a different sleep center. Everything went well, it was much nicer environment than my original sleep center.
On arrival I had a great conversation with the titrator, who greeted me as "Ahh a slim one" she was fantastic, she listened and we had a good two way conversation about my concerns including my centrals in the 2nd half of the night. She explained that she would try first to deal with them with Bi-Pap first if she shes observes any that not related to movement or arousal's.
I noticed the machine was a very large Resmed VPAP ST, i joked i with her than she better make sure that the machine was still there in the morning after i leave
I used my Swift LT and PapCap and I was asleep around 10-10.30 pm after 10mg of ambien and woke up at 5.30am.
In the morning she explained I had a good deep sleep and that the doctor would be able go go over my results in a few days. I managed to get some information from her as I think she was enjoying the fact that i was interested in her job. Given that i was previously titrated at 7cm I was surprised to hear that she had me started a 5cm and got almost as high as 6cm using an EPR of 1. She only saw expected centrals on arousal/wakenings, definitely no need for BiPap.
I woke up slightly hazy but was as sharp as anything after a shower and an orange juice. I stopped for breakfast on the way home and sat and watched the sun rise over the mountains. Its been a long time since I've experienced this part of the day. I feel alive and alert and I hope it lasts the day.
Thanks for the sleep center recommendation Carrie (sleeplessinaz)
So far so good. They even gave me a free Swift LT mask on the way out.
I'm going to try 5cm-6cm with EPR of 1 on my S9 tonight.
I hope I feel as good tomorrow as I do this morning.
The journey continues ....

Watch that OJ
I am glad you had such a good experience, however you should know better about diet.
A glass of orange juice consumed without any protein or fat causes an unhealthy spike in blood sugar, followed by an unhealthy spike in insulin, and eventually an unhealthy dip in blood sugar. This is even true for people with healthy metabolisms and healthy insulin receptors.
Fruit juice consumption should be restricted and always accompanied with fat and protein intake.

Night Monkey, I always have 4oz OJ for breakfast with oatmeal made with skim milk and half banana so I guess there's no protein or fat there, right? My blood sugar levels are slightly elevated but within normal range so I try to watch my sugar intake.

I had sausage, eggs and potatoes soon after the OJ.
I had no idea my titration post would turn into a orange juice debate

Dori, it looks like there's at least a little protein and a small amount of fat in all those things:
Banana (they're probably talking about a whole banana:)

Quaker Quick Oats:

skim milk:

juice from raw orange:
http://nutritiondata.self.com/facts/fru ... ces/1971/2
If the orange juice is from chilled concentrate, there's no fat:

Bob, what a good sleep lab you found! Smart sleep tech there.

Re: I love Titrations
Hi Uncle Bob--wow--I am so happy to hear that you had such a great experience at the Valley Sleep Center. They are pretty great people there!! AND you got a free Swift LT mask in the way out--wooohoo for you--LOL! KUDOS!!
I was at a "7" before the titration that I had in December and they bummped me up to an "8". I got an S9 Auto set out of it from my Primary Care doctor---:). They set it for a range from 6 to 10------and the autoset says I am at 9.6 most of the time!!! HOLY COW!!! I have to say that I did not sleep hardly at all at my titration as I did not take my ambian ---like I should have. I had lots of arrousals that night and was just not sleeping like I should have--:(. plus I lost like 30 pounds so I wonder if that is the reason for the raise in pressure?
I am so glad that you had a good titration uncle Bob---:)

Re: I love Titrations
I am with NightMonkey on this subject and would never drink a glass of OJ or any fruit juice with any meal. The glycemic index for fruit juices is very high.
For breakfast I put a small amount of crushed ice and one ounce of OJ in a 16 or 20 ounce glass. Then I fill the glass with diet tonic water - delicious and refreshing and keeps the fructose intake way down. I eat one XL egg, three ounces of walnuts and three ounces of bran buds.
That gives me protein, fat, fiber, omega-3, and hydration. I no longer need a mid-morning snack and if lunch is late there is no panic.
For lunch I also make sure to get at least four ounces of protein and some fat along with a veggie or two and maybe a small fruit.
I buy the smallest apples and the smallest bananas in the produce department and they make a great dessert substitute after a meal while bringing minimal carbs and blood-sugar effect. I also buy small oranges like Clementines or small Tangelos for alternating as desserts with the apples. Apricots, nectarines, peaches, and cantaloupes are also good in small amounts. My weakness is watermelon - I like huge slices.
Glad your titration went well!

Re: I love Titrations
How is your pressure so low?? Can't you maybe attach a fan to your headboard and aim it at your face to get a pressure of 5 or 6?! It is interesting that one poster once said that Stanford doesn't titrate to less than 7cm (or some such thing, that I'm likely misquoting).

Besides which I kinda have a soft spot in my heart for a good RPSGT as the highest percentge of the most caring and dedicated part of the sleep profession. The dedicated and caring seems to drop off precariously as soon as you add the RRTs and sleep doctors.

Re: I love Titrations
Well who would have believed 5cm-6cm with EPR of 1
It actually worked well on my S9 last night, AHI of 0.2, a couple of centrals. But i feel alert and rested.
5 cm is really low, you have to be relaxed and slow breathing before you start. No running up the stairs and jumping into bed.
Looking at my stats my 95% pressure was 5.4cm and the maximum was 5.8cm.
Exactly as my friendly titrator said, starting at 5 and almost got to 6cm.
No Orange juice for me this morning, just some beers at a lunch time BBQ

Re: I love Titrations
Did they titrate to eliminate RERAs? How was the outcome of this titration different than the one before? If I remember correctly, you were doing this because you were still tired on your previous pressure, right?
