Headaches and always tired ...

Hi Luis... first, please go to the User Control Panel and your Profile--> Edit Equipment and choose your gear from the drop down menus. The choose to show it in TEXT, not pictures. THis will post your equipment to each post you write.

I am no expert, and you are using different software than I use....but it appears that tightening up your pressure range may help a bit. Your overall 90th %tile pressure is 8.5 so I would try setting the range to 7-13 and see how that works for you. Your one night with the very high pressure of 20 also had large leaks so I would discount that all together. Your AHI is pretty good, but could still be lowered a bit more.

If you don't know how to adjust your machine's settings, once you post your machine type, model, etc. We can help you get the clinician's manual so you can get under the hood and make adjustments.

Others, who are much more experienced than I, will be along to help as soon as you tell us the info on your machine, mask, etc.

It looks to me like you need to raise your minimum pressure quite a bit. Maybe start with it at 7 or 8 cm and see how things are in a week. I would also set the maximum at 14 or 15 cm for a week to start. I see these are the pressure settings suggested by Emilia. I agree with her.

I think you are taking too long to get to a optimum pressure for you so that events are not being dealt with so you are not getting optimum therapy for much of the night, thus having headaches. The changes in pressures all night may be keeping you from getting restful sleep, thus you are alway sleepy and need a narrower range.

It appears the machine is the Respironics PR System One APAP with AFlex.

With a minimum of only 4 cm and the max of 20 cm any number of things could be contributing to problems. Most common one being the 4 cm minimum is not enough to deal with events quick enough. I would think a bump up to 7 or 8 cm minimum would be a good place to start.

I see that your nights of sleep are often short and fragmented. Many people find that the variations you see in pressure will affect their quality of sleep. For that reason it is often suggested that the maximum pressure be lowered some to see if that helps.
If the pressure increases wake you, then yes, for sure drop that maximum down a bit.

Overall, I would suspect that the fragmented sleep and lower number of hours in sleep, will affect how you feel the next day. I know that when I first started on the machine I had other issues that caused fragmented sleep and still felt rather crappy the next day. Take a close look at your overall sleep hygiene to see if that can be improved too.

With a minimum of 4 cm, that also could be affecting things, just from a comfort perspective. I would feel smothered at such a low setting and that alone would affect my overall sleep. I have always said that just a low AHI doesn't indicate an ideal thing because if we don't get good "sleep" because of other things, we can't expect the therapy to be a magic wand and fix other issues.

Even a window of 6 cm to 12 cm could be more comfortable overall and still be effective. It doesn't appear that you need much more pressure to treat events. Might also consider running in CPAP mode with maybe 6 or 7 fixed pressure and see what happens.

I think that would be a good start. Give it a few nights and see what your reports show then. Don't expect a miracle overnight though. I suspect that you probably have more than one thing going on that affects your quality of sleep.

Thanks . Forget to teel a feel during the day a permanent beep on my ears... could be related to CPAP ?

What should be the AHI that we all loking for ?

Don't know about the ears.

The sleep medicine profession says that an AHI of 5 or under is good therapy. Most of us feels better at less than that. For me, I feel best when my AHI is below 3.

Uncertain as to what the "beep" may signify. I don't really understand what you are saying.

Most of your AHI is less than 5 and that is considered to be effective. Some of your higher AHI numbers also show with the machine's pressure going all over the place and some pretty good leaks.

What you need to look for besides the AHI, would be a more consistent pressure line and leak line. With APAP you will have some pressure increase but yours are really all over the place. Some of your leak lines are also all over the place. While we cannot expect a perfect straight line it needs to be straighter than it is. I noticed that on some of your reports there is a higher AHI and it seems to correlate with the pressure fluctuations and leaks.

It appears that you also need to work on the leaks too. So see if you can get the mask better fitted.

There is a condition called tinnitus that produces a ringing in the ears.
