Bipap Auto SV setting advice

Post by grandpap » Thu Mar 04, 2010 12:36 pm

I recently purchased a used auto sv with one hour use on it(lucky me).I am self titrating(long story)and was wondering if someone could suggest a starting point to find my optimum settings.I do not have serious health issues but am pretty sure that I have undiagnosed compsass.I tried to convince the sleep doc to look into it but he just talked about losing weight,so here I am on my own. Here are my settings:
EPAP:11 cmH2O
IPAP Min:14 cmH2O
IPAP Max: 19 cmH2O
Backup rate: auto
Rise Time: 0.6 (second)(seems a little long)
These are not arbitrary numbers,I have several months of data that i used to determine lows and highs.I actually set my IPAPmax a little low to reach an optimum.I was planning on inching my IPAPmax up as needed during self-titration.I am also low on EPAP/IPAPmin ,I determined this because my VS is pretty bad.
At these settings my AHI is 16 and they cluster around pressure drops.I also have issues with fls and nrs as pressure exceeds 19 h2ocm.(bipap auto data)
If folks don't mind I would like some pointers on setup and no I do not want to work with my sleep doc.It only took quitting cpap and 15 years to covince someone that I needed a bipap.
Btw,I woke this morning extremely tired and a headache, that hasn't happened in quite a while.I also experienced a "thumping" of the mask at the change between IPAP and EPAP?Thanks

I would highly recommend having a sleep study done before using that machine. As many times people have posted to high with your IPAP can cause you to have centrals. This machine was built for people that have OSA as well as centrals.I know,I use this machine since I was diagnosed with Complex Apnea!

Sleep study titrated me at 12 straight cpap, even sleep doc couldn't give a good answer for that one. When I gave up on him I was up to a cpap max of 22 and I could hardly keep my mask on my face.
IPAPmax of 30 h2ocm Wes,good lord!

I hope you have software to monitor your therapy, especially with this beast!

To get rid of the tired feeling and the headache try dropping EPAP to 10cmH20, IPAP Min to 13cmH20, and RT to .3.

The thumping at the mask during IPAP/EPAP switch is inherent to the device. The only way to get rid of the annoying IPAP/EPAP switch is to purchase the ResMed Adapt ASV.

Thanks Banned for the info.I appreciate you taking the time to answer.I am fairly close to a 10 and 13 now which brings up the issue of high vibratory snore. When the numbers get that low I tend to have a tough night..I will try the settings especially the .3
I would like to have the adapt SV but can't afford it.I was lucky to enough to find this unit at an estate sale fo 220$

My Adapt SV doesn't really thump between pressures & the settings are close to what you are mentioning. But I am aware that these machines have different software levels. I know that Bev's SV (OuttaSync) has a noticeably different software level to mine.

#2 - Banned, also note that Grandpap may have CompSA (is showing the symptoms) so be cautious on any epap/ipap gap suggestions

Michael

I got mine from cpapauctions.com

of late they are at very good prices BUT make sure you have a prescription registered for an ASV machine before you try to bid.
You will need to convince a doc that an ASV will be of benefit to you.

Good luck

Thank you DSM! I will watch, and work on the docs in the meantime!

Thedocs both agree that I have Complex Sleep Apnea, and that the untreated centrals are the problem with my current CPAP setup......

Slow, but getting there .....
Cheers,
Michael
