raised pressure UUUUGGGGHHH!!!

Post by cotech50 » Fri Jan 01, 2010 7:04 am

Pressure was rased to titration report 11-14. I was 7-15 agreed to 9-14 first. Felt good at 9-14 thought 11-14 would be a breeze. Find I am starting to fight the machine have to relax and focus on falling asleep, only 2 nights so far first only 3.5 could not get back to sleep last night woke several times not sure what was going on got 5-6 b4 getting out of bed. I guess object was to get my low closer to my titrated pressure of 11 and 12. 11 snoring stopped but some arousals pressure was increased to 12 only 39min there inconclusive. I may have to start using ramp or increase flex to max as a few times thought I would have to take it off and I do not want to get that started. I am going forward not backward.

I assume there are several opinions of what your low pressure objective should be compared to your optimal pressure. My ahi's have been mostly below 2 on 9-14 with best 0.7. Most nights higher percentage of night was on cm9, some 10,11,12, few 13 and 14. I see ahi info spead out over pressures I cannot see a silver bullet so to speak as I may have an ahi lower at 11 one night but maybe higher there the next. I will need to give this new setting a chance just to get enough numbers to make a judgement. Seems if i am below ahi 5 all are happy, I need suggestions on what i need to really look at in these numbers, I have yet to figure out this vibratory snore I am not sure what it is reporting very erratic numbers there. Things were getting good at 9-14 was starting to get some 7+ nights I hope I can get adjusted to this new setting I will give it some more time.

Hope all are having a good new year your thoughts and suggestions will be appreciated.

Which positions are you sleep in? Do you know from your sleep studies how different positions affect your condition and your pressure requirements?

I did not have arousals I had flow limitations at 11 and 12. Very little supine sleep. I sleep on side, I find if I am on back mask leaks at chin when jaw relaxes that is why i sleep on side. Have found also with increased pressure I have more mask issues that could be an issue. FL does not seem to be an issue.

Large leak has gone to 6% b4 0 definitely different issues start occurring at higher pressures!!!!!! Could this be the so called SNOWBALL EFFECT!

higher pressures can cause centrals that's why so many people say don't go too high without a titration. But you were titrated to that pressure if I am reading this correctly. If you are supposed to increase your pressure up to the titrated pressure and are having problems with it, don't jump right up. Slowly and gradually. I have heard docs, dme's and people on here say to increase slowly give it a few days on that pressure then go up a little more. Some people have made a change up one and stayed at that a week then gone up another one.

If you feel your having problems with the higher pressure, try increasing it a little and give yourself some time at that pressure.

The RT did this I went up 2cm from 9 to 11 based on what the Pulmonologist suggested. I will give this at least this week to get used to and keep checking avg AHI is 2 which is fine with me.
