I got into a bit of a discussion w/Mike, one of the Admins at the ASAA apnea support forum. And am not sure I was as diplomatic as I should have been. Ah well. "I" thought I was tactful. But I was just beginning to get warmed up to the subject when I thought it best to quit whilst I was ahead.
I can understand their restraints in that forum. BUT ......

Too bad he's clueless about diabetes, too. I actually got more information about Sleep Apnea from my doctors (which was ZERO) than I did about about diabetes (less than ZERO). He just looked at my blood draw report and said "You have diabetes". (and wrote out a prescription for Glucophage, which it turned out didn't agree with me)
If it wasn't for the Internet forums, the people with medical conditions would still be in the dark......like they were BEFORE the Internet.
Well, if he feels like you're winning the argument, he'll lock the thread.....after he gets the last word.

So having read that argument (amusing) and being new at this that leads me to the following questions:
1) How difficult is it going to be for me to get an APAP machine if my scrip does not say APAP.
2) How difficult is it going to be for me to a) get into the hidden menus and b) download the data from my new machine?

Needsdecaf - I made an appointment with my sleep doc, told her to write "APAP" on a NEW scrip for me, and brought along 27 8x10 color glossy pictures with a paragraph on the back of each one, 'splainin' what each one was, and the circles and arrows.... Oh wait... That was "Alice's Restaurant" I was channeling there... Really have to check my meds in the AM...
ANYWAY, she never even wanted to see it. She told me to let me know if she needed to talk to the DME or my insurance for me. She told me she does that all day long. She also told me she trusts me to do self-titration as it is my intention to continuously monitor myself as I lose weight.
It's possible your doc won't be as reasonable as mine was. And not knowing that, you should go in armed to the teeth. But pray the doc has a brain. Several of us here have been successful in making the case.

There is merit in the approach, however. It completely changes the power dynamics when the patient convincingly demonstrates overwhelming knowledge on the subject.
When I brought in full-color printouts of more than a week of oximeter results, the sleep doc didn't really have much to say. He did however write the machine prescription per my request ... which was an auto-BiPAP.

The irony (or should I say hypocrisy) of Mike riding his "high horse" over there is that he started out here in early 2005. Got himself a new REMstar Auto and Encore Pro software and was trying to figure out his own pressure settings. Now he an evangelist preacher about the harm that "air" can do to you.
If you have the stomach to do so, go up to the Search function and look up all the posts by "Mikesus". Go to the oldest and start reading forward.
I'd like to know how many people have actually been "harmed" (or killed) by tweaking their own pressures as opposed to the mis-titrated pressures prescribed by the doctors.
Don't tell me that optimizing one's OWN therapy is "DEADLY"!
If a doctor prescribes you an APAP set from 4 - 20 cm, THAT wouldn't kill you but it wouldn't be effective therapy, either. Various circumstances could prevent you from getting the proper pressure and other events could drive the pressure beyond what the optimum is, too. It still won't kill you, but you'll probably feel like crap.....like before therapy.

It's a whole lot better than being without therapy ...
My opinion is that the number of folks being effectively treated for OSA would more than double nearly overnight if APAP's became the standard of treatment, without so much as a single PSG being required.
Yeah, some folks would be better served with conventional treatment, but a whole boatload more would be better served by getting them started immediately on treatment.
