Newly Diagnosed, Check My Oscar

Hi all,
New member here. Newly Diagnosed with well over 40 events per hour. Being the impatient person I am, I bought a new/used Resmed Airsence 10 locally. When I get my script I'll see what insurance offers but wanted to get started right away and this is the machine I wanted anyway. So hooked it up and without knowing what Doc is going to prescribe I tried the settings you'll see in my Oscar link.

Was hoping for some input, looks like I picked good APAP numbers, but says AHI was still over 5. What say you guys?

you might want to increase the minimum from 7 to 8. it starts at 7 but never really goes back down,

stay at 8 for a few more nights. it can take awhile to see the full benifits.

Sage advice from Zonker! That's a wonderful improvement.

The CAs probably just indicate brief periods of arousal from sleep or from one sleep stage to another. They may well diminish on their own as your body gets used to the PAP experience. I would just bracket them for now and stick with your new settings.

And let me mention how impressed I am that you got your own machine, got Oscar, and came to the forum. Not only that -- you're sleeping a full night with your mask and machine!

Going forward, of course keep an eye on the numbers, but also keep track of how you feel subjectively. Did you feel as though you woke up frequently the previous night? Are you feeling rested during the day? Were there other relevant variables in play (evening alcohol, big meal, pesky pets, etc)?

Thanks, yes going to keep it where it is at a few nights. Woke up this morning feeling much more refreshed and got up without hitting snooze, jumping out of bed and running out the door. Actually had time to make and drink a cup of coffee and sit for a few. Hope this trend continues and improves.

No big meals, working on a healthier lifestyle last 4 months. Down just over 30 pounds, only about 30 to go to my goal. No alcohol, meds etc. Saving my binge drinking and eating for cruises, 119 days till the next one and we just got back from one (gained 10 pounds and averaged 17 drinks a day, IT WAS AWESOME) LOL.

Thanks for the suggestions and support. I will post updated images in a few days unless anyone asks to see before.

Good morning all,

So the last couple days I do feel a bit better during the day. Still a little trouble sleeping but AHI is below 5. Seem to get a lot of CA's but maybe that could be because I am tossing and turning at that time, or maybe I am tossing and turning because of the CA? Also still more OA's than I would like, but better that before APAP!

I am going to link yesterday's Oscar images, hopefully I can get some comments on it. There were some hyponea's I don't like as well in the clusters and noticed my pressure would go up over 12. I wore my apple watch to bed and it says my O2 dropped to a min of 95 overnight so that is also much better than before, it would go under 90. As you can see my pressure is set to 8 - 15, once it raises over 9 it really never goes back down to 8, I was thinking of changing it to 9 min, sound right? The pressure does not bother me at all.

You will also notice a afternoon nap in there and for the nap I tried a different nasal mask (did not like it, have to adjust it to much) Here is the link: https://imgur.com/a/ebTD6fB

Looking at it again, pressure does go back close to 8 and as soon as it does it appears I started having events again. So again, I am thinking a higher a minimum.

Do you know how to use SleepHQ? Using it would let us zoom in on those centrals/CAs and see if they are maybe the cause of the tossing and turning (arousals) or if they are the result of the arousal happening. Those centrals could be a symptom of poor sleep quality but not necessarily the cause of the poor sleep quality.

Assuming those OAs and hyponeas are the real deal and not related to arousals...then yes a higher minimum is worth trying but if you weren't sound asleep when they got flagged then a higher minimum probably wouldn't help much.
Again if we can zoom in on the flow rate we can usually see if you were asleep or not.

To try to fix any problem we first have to try to identify the problem and/or the cause of the problem.

Thanks. That worked quite well.

IMHO the bulk of your CA/central flags are post arousal and not asleep centrals.
Some of the OAs as well seem to be post arousal.
I don't see the arousals being related to airway flow reductions though. Seems like you are asleep and then have some sort of spontaneous arousal and THEN you get irregular breathing and THEN the machine flags that arousal/irregular breathing as some sort of apnea event (and most of them centrals which is common).

I didn't go through the entire night though.

Below are some examples....post arousal OA flagging as well as post arousal central/CA flagging.

arousal with no flag....I circled the arousal breathing in red. Compare those breaths to the nice even breathing prior to the circle where you were likely asleep.

Central flag but after obvious arousal breathing then you go right back to sleep. Turn over in bed and hold your breath central??? Good chance of it. What we can't see or know is exactly what caused the very brief arousal.
