Need help with wife's pressures

Post by Iamstumped » Wed Jul 06, 2022 9:54 am

My wife has just started CPAP therapy, (again). She is using the AirCurve 10, mode S, IPAP 16 cmh2o EPAP 12 cmh2o. Using OSCAR, I see that her machine is constantly at the max level every time that she uses her machine. Is that normal, or do we need to increase the IPAP pressure some?

What are the max and min settings? What's her typical AHI and its components? What's her typical leak line?

For some unknown reason, I could not attach OSCAR screenshot.

Put it on a host site and post the link here. I use imgur.com.

AirCurve 10, mode S, IPAP 16 cmh2o EPAP 12 cmh2o. Using OSCAR, I see that her machine is constantly at the max level every time that she uses her machine.
Mode S on the AirCurve 10 is a fixed bilevel mode.
IPAP 16 and EPAP 12 means PS is 4....it's never going to change.
Max is 16 IPAP and it's always going to be at max when in S mode because the pressures won't/can't vary at all beyond what PS lets it do and PS is fixed as well.

So if indeed using the S mode....what you see is normal results in terms of what the machine can do in fixed (S) mode.

Confirmed by the OSCAR report.
S mode and what you see is actually very normal and to be expected when using S mode.
She would need to be in bilevel auto mode for the IPAP/EPAP to ever do anything differently and would require different settings as well.

Do you believe that her settings should be changed?

Probably.

I don't like what I see there both in terms of centrals and OAs but please don't ask me what to change things to because at this point there is so much that I don't know about her history and how she came to be prescribed the machine and why she is now "trying it again" which begs the question as to why she quit.

I don't like making suggestions without more information if at all possible. Flying blind and I just don't like flying blind.

Starting with original sleep study results if available.

Why the fragmented sessions?
Why does she wake up?
Does she report even being able to sleep at all????
Take any medications????

Lots we don't know and what we do know is very limited at this point.

As for trying it again, she tried CPAP about three times in the past. She does not like the air blowing in her face.
She is now using the Airfit N30i nasal mask.
As for the fragmented sleep, she has to go to the restroom multiple times each night.
As for meds, she does have about 7 different prescriptions that she takes daily.
She says that she falls asleep within minutes of laying down.
This machine was setup like this by the DME that we got this machine from.
She has never gotten her sleep test results. I will try to get her to request them.

Did she ever have a sleep study done in a lab where they put the mask on her and used a machine to find optimal pressures to be used?

Yes, we both had a sleep study done in March 2022.
I've got my sleep study results, just got to get them posted on here.
I'm trying to get her to request her results and her prescription, and then to let me post them here, so that we can get some help.
The DME that we got her AC 10 from, says that everything (her machine readings) are fine. I do not believe that garbage. I'm seeing a bunch of events on OSCAR, when I import her SD Card.
