Hello,

First of all, I am sorry for posting this since I am sure the same thing has been posted many many times. I have tried searching for a similar post before writing this, but I am new to the forum and got confused/lost amongst so much information/so many posts my search has returned. Also, my brain fog is getting the best of me. Of all the people in the world, I am sure you're the ones that can understand this last point :)

This is my second try at CPAP, so I am not a complete newbie, but the last try has failed many years ago, and although I have some basic knowledge, I am kind of starting from scratch. Last time I had a full face mask (since I am a mouth breather) and always woke up with a dry mouth in the middle of the night, and couldn't sleep unless I took my mask off. I have tried a few things for a few years but it hasn't really worked for me, so I kind of gave up and sold my CPAP in order to help pay for a custom-made mouth guard for sleep apnea, as an alternative. The mouth guard does help (I sleep better with it than without it), but it hasn't been amazing either: I still wake up feeling somewhat tired, moody and low energy, and sometimes with headache.

Dry mouth/mouth breathing has always been a problem for me, and finally this year I discovered mouth taping and it literally changed my life. My sleep improved considerably, although still not great. For a few weeks this summer I woke up feeling refreshed and energized, and I had a glimpse of what my life could actually be like, which definitely pushed me to give CPAP a try again. I believe that with mouth taping most of the issues from my first try will be solved or managed.

as a side note, I have done pretty much all I can to ensure my nose is clear to try to solve the mouth breathing issue, and although everything I have done definitely helped, only mouth taping really ensures I breath through my nose. Things I have done include: deviated septum corrected (surgery), turbinate reduction, saline rinse twice a day with pulmicort prescribed by my ENT to help with nose swelling/inflammation. I also used to have recurring sinus infections (about 4x per year), both ON and OFF CPAP (so not CPAP related) and I have undergone sinusectomy surgery to help with that, and it did help somewhat, but every now and then I still have sinus infections (probably a couple of times a year now, give or take).

So I just bought a RESMED 10 autoset, with a nose pillow mask, while also wearing the mouth guard (it helps open my airway/keep my tongue in place). I am not loving this mask, and I am considering buying a nose mask. I am open to keep trying the nose pillow, and maybe I didn't give it enough try, but I also would like to buy a nose mask and give that a try. How do I choose a good nose mask that I know will fit me alright ? I am a side sleeper if that makes a difference.

Also, as a side question: Is it normal/ok to mouth tape using a full face mask ? I imagine I'll have to get one of those too, since at some point I will have a sinus infection and I won't be able to use a nasal mask/pillow then. Reality is maybe I won't even be able to use the full face mask... when I have a sinus infection I can't bear CPAP... anyone else on the same boat of the sinus infection has any tips for this ?


Sorry, I didn't mean to tell you the story of my life, but I also wanted to make sure I included any important/relevant info, as I am sure people will suggest some of the things I have already tried.

My full face mask lies forgotten since I bought it.
Breathe right strips have allowed me to use my nasal pillows every night.
Cpap has improved my immunity so that I have not had a severe sinus infection
for several years.

Sometimes the nose pillows (the ones that sit inside the nose) have more resistance you are trying to breathe against, trying just a nasal cushion mask may be a good idea. Being a side sleeper isn't the most important issue when choosing a nasal mask, so much as how restless of a sleeper you are. Some of the cushion masks have a tendency to shift under your nose easier than the pillow mask if you move around a lot.

If you are using a full face mask and taping and have a sinus infection, you will not be able to tolerate the CPAP, you won't get enough pressure through that congestion with you having your mouth taped. I certainly do not recommend that. The whole point of the full face mask is for those times you can't breathe comfortably through your nose so you are opening your mouth and still getting your therapy. You may try upping your heat on the tube and also upping the humidity during those times of higher congestion as the added moisture can help thin the mucus.

Thanks for your reply/suggestion. I have tried nasal strips, but they didn't really work well for me because I have oily skin. Nose dilators (plastic piece that goes inside the nose) did work well for me, but I haven't used them with the CPAP. They may not work with nasal pillows since they also go inside the nose, but maybe with nasal mas they do...

Thanks for your reply! I appreciate you clarifying the difference between nose pillows and nasal cushion. I believe I found a replacement part that is a nasal cushion and works for my mask, and it costs about $35, so I will give that a try! Worse comes to worse, $35 is not that much money to spend and try something new.

My reference to a full face mask was in case I have a sinus infection, I can use that. With the nose congestion that a sinus infection brings, I would NOT be mouth taping in this occasion. But I think I will go at this one step at a time and try to figure out a mask that helps me breathe through my nose first, and when I get to a sinus infection I will figure that part out...!

OP did not tell us which pillow mask was used. There are several out there, and some work better than others. What works well for one user may not work well for another. It's not about "right" or "wrong", it's about finding what works for you. For example, I did not do well with the P10 because I felt it was portruding too far into my nose. I do much better with the Brevida. The protruding feeling is gone, especially when using the Small size pillow. Medium size Brevida pillow does not work as well for me, even though I use a Medium in some other masks, so my advice would be to try some other sizes and brands before abandoning pillows. They are the easiest to seal.

When you speak of masks, please specify the exact model. There are many models of nasal pillows and all other types of masks.

Do what you need to be successful with CPAP. I use mouth tape under my FFM. Works for me.

WTH? That's for asthmatics. It's inhaled by mouth and works primarily in the lungs. Do you have asthma?

For nasal congestion, Flonase and Nasacort (and their generics) are prescribed.

As far as selecting a mask, go to the forum sponsor's page, cpaptalk.com, and look at photos, specs, ratings, and reviews of many masks. If you see something interesting, you could come back to this thread and ask questions.

My Bad, I failed to include that info! I am using the Philips DreamWear Gel Nasal Pillows Fitpack

I don't have asthma, but I do have sinus inflammation. My pulmicort is diluted in my sinus rinse, so it is a different use than normally. Anyway, I was prescribed by an ENT to reduce inflammation and swelling in my sinuses, and it's been helping.

I have also found that some retailers accept exchanges within 30 days, so I may do that. Buy one, try it out and if that doesn't work for me I can exchange for another model, until I find something that does work... But I will definitely refer to the information you gave me before buying, thanks!

Hey CB - I used the Dreamwear Nose Guide and that put me at Medium. I just got a Brevida (2nd Night) and am using the Med-Large. Wondering if trying a XS-S might be worth trying but it is $25 plus shipping to find out. XS-S made a big difference for you? Any advice for first time Brevida users? Feel generally good and seals within 95% 1 liter leak rate. Just some pressure discomfort. Wondering if a smaller size would help. Any issue with the strap bothering your ear when sleeping on the side? I have been keeping it up high towards the top of my head. I did not like the Dreamware but I am liking this much better.

Just curious, but you never mentioned using the humidifier... or how much, for how long, at what setting, etc. I ask because I think a lot of people use them too much and end up with sinus problems... or have you already considered that?

Sorry if I didn't mention, but I do use a humidifier. I use the "auto" setting.

I already had a previous sinus problem even before using CPAP.
