I’ve been using apap with limited benefit for several years now. My pulmonary doctor just basically said it takes time to feel better and it hasn’t. I even had multi level surgery , thinking that I’m not Cpap tolerant and my ahi went from 10 pre surgery
To 20 post surgery. If anyone knows an excellent sleep apnea surgeon in south Florida I’d like to see if perhaps there’s an adjustment to my tongue base suspension suture that can fix my situation because months after surgery ,
Complaining about being tired, after my surgeon said it can’t be. I finally took another sleep study and was horrified to see
My ahi double. He performed
a
DISE and said my jaw falls back and even though my tounge
Is suspended. I asked if I can just use my MAD and he said he doesn’t know. I no longer trust the surgeon that performed the surgery because he’s pushing a jaw surgery now without trying to make what he did work(I paid out of pocket for all of it) My palette was modified and it seems to be more open. Even other Ent’s said it looks
Good but obviously my apnea is worse now.
Until I pursue further surgically, i still feel horrible and Need to get my cpap to work
For now, I have purchased an innova pulse oximeter recently to see what’s going on and sure enough I still have many low o2 saturation during apnea episodes.
I’ve Recorded my o2 for 2 nights now. 1st night the minimum pressure was set to 6.4 and an ahi of 6 as per resmed but 10 as per o2 software.
Last night I decided to increase the minimum pressure by 1cm to try to get better results and got a ahi of 4.5 according to resmed but 13.4 according to o2 software.
I also installed sleepy head for one night but don’t know how to attach the file.
I really appreciate any help.

The scoring parameters that are the default with the oximeter are usually too sensitive and give rise to many more events. I was instructed to change the scoring to a change of 15 BPM over 30 seconds. That is not perfect but it will reduce a lot of the "noise."
Give that a try and see of your oximeter and your CPAP number get closer to agreement.
