New ResMed Aircurve V10 Auto

So I purchased a used Aircurve V10Auto after using a Respironics BiPap Pro with over 35000 hours on it for 13 years. I listed the settings below and wonder if there are any recommendations to adjust--BiPap pro setting was 12/8 and worked fine for me for years but have no data to support it.


Settings are as follows:


Mode:VAuto
Max Ipap:12
Min Epap:8
PS:3

TiMax:2.0
TiMin:.3
Trigger:Med

Cycle:low

Ramp:15 Min
Start Epap:4.0
Climate Control:auto
Tube Temp:auto


Do these setting sound correct for the first night?

So, your old settings were a EPAP of 8 and IPAP of 12. which was a PS of 4.

If you want to take advantage of the VAuto, then set it up like this:

MinEPAP 8
MaxIPAP 25
PS 4

The other settings are fine for starting out.

Your settings on the VAuto have PS set at 3 but in the past it seems you were using PS=4
Are you sure you want to use a ramp?

The ramp will start off at 7/4 (because start EPAP is 4 and then add 3 for PS to get the IPAP value)
After the ramp it will climb to 11/8 (because min EPAP is 8 and then add 3 for PS to get the IPAP value)
The machine will not go higher than 12/9 (because max IPAP is 12 and PS is 3)

If you wanted to get as close as possible to 12/8 and effectively turn off any automatic up/down drifts then you could try..

1) Ramp off
2) Max IPAP 12
3) Min EPAP 8
4) PS=4

If, on the other hand, you want to give a chance for the pressures to go higher if the machine thinks it needs to you could try (for example)

1) Ramp off
2) Max IPAP 15 (for example)
3) Min EPAP 8
4) PS=4

This will start things off at 12/8 (based on the min EPAP plus PS) and allow the pressures to rise to 15/11 if the machine wants to (based on the max IPAP minus PS).

I think I got the math right.

His settings are similar but would allow the pressures to go even higher than the example I gave.

After you've used it for a couple of nights, look at the data in OSCAR and use that information to narrow down the min and max ranges if you want tighter control.
Otherwise just let the machine do its thing.

Got my first night of Oscar in. I see a lot of apneas when I first started using and was trying to get adjusted to the machine but after the first 1.5 hours things seemed to smooth out. Notice some mask leaks also. Does anyone have any comments?

Optimizing the Min pressure: absolutely.

But "narrowing the max range for tighter control" is nonsense.

If the machine doesn't need to go higher, it won't. And if it does need to go higher, it can't because you've handcuffed it. There are valid reasons to limit the maximum pressure but you're arbitrarily suggesting to limit for no other reason than "tighter control".

Yes, we need to fix those mask leaks.

Your AHI is huge and the absolute majority flagged are unknown because the leaving was too large for the machine to reliably determine whether they were central or obstructive.

You're also posting way too much stuff.

Turn off the pie chart,
post Flow Rate, Mask Pressure or Pressure, Leak Rate, Snore, and Flow Limitations.

Your Flow Limitation and Snores are pretty clean, so I'm guessing your minimum pressure is probably OK, but with leaks as large as yours flagging and determining Apnea type just isn't reliable, so neither can analysis.

Some people are very sensitive to extremely high pressures and, for them, they are willing to make a trade-off on the max pressure even if this means letting a few apneas sneak through.
The sentence also said "if" you want tighter control.. and was rapidly followed up by "let the machine do its thing".

Then you should have said, After you've used it for a couple of nights, look at the data in OSCAR and use that information to narrow down the min and narrow the max ranges if you are sensitive to higher pressures."

Arbitrarily lowering the the max pressure, which is the advice you suggested, for no other reason than wanting "tighter control" is bad advice. Qualifying that you said "if" doesn't make it any better.

Hi All,


After 2 nights with major leaks on my F30i mask, I switched to my Swift LT nasal pillows and achieved a reasonable night without leakage. Have posted last nights Oscar report. Only issue I had was exhalation still seems a little hard for me, fighting to breathe out. Any recommendations for a lower setting that would help this?
One thing to note--Oscar reports my Climate Control as manual however it is set to Auto in the unit--possible bug?

I'm really amazed that things just keep getting better. My AHI index went from 12.9 to 6.6 to 5.5 to 2.3 to 1.3 to 1.1 to .55 last night. I guess this is meant as encouragement to folks trying to find the right setting--small tweaks do indeed make for large improvements! So happy I bought the new ResMed Vauto and retired my old Respironics with 35000 hours on it.
