Hmmm. not seeing any improvement so far. Think I just need to get used to the new settings ?

Go to a minimum of 13, you're still struggling for air a lot of the time.

Well, the whole night, FLs seem to be what's mainly driving pressure increases, It's not apneas, hypos or snores.

The increases in minimum pressure seem to be helping even if they are thus far subtle. There is less sustained clumping of steep desaturations in your last SpO2 graph until about the last hour of the recording where it gets a bit nasty. You have justification to keep increasing the pressure since you have an ugly flow limitation graph and still some breathing events. The increases in pressure are still being done by the book so to speak :).

And keep in mind that with classic bilevel therapy in a clinical setting, EPAP is where oxygenation happens, and PS is for ventilation, aka removal of CO2. So if SpO2 is low, they raise EPAP.

No real imporovements to speak of yet guys....is it something that will improve as i get used to the new settings ?

Do you have a new corresponding SpO2 chart? The machine still wants to go higher, it doesn't spend much time at 13. I would keep raising the min pressure or drop down the EPR so there is less of a drop in effective pressure. A drop in EPR may have an effect on some of the data statistics but I am afraid it's probably just an experiment. And once you reach a min of 15 or so it may be worth looking at bi-level if it becomes uncomfortable.
