I experimented with just about every oral appliance on the market before accepting that I needed xPAP.
Tongue Advancement Devices were indeed, for me, far more effective than Mandibular Advancement Devices. However, it was hard to keep in, and as others have said, it didn’t address the whole problem.
I got the best results by combining a WoodyKnows Nasal Dilator, a Good Morning Snore Solution TAD, and a Z-Quiet MAD. The Z-Quiet was not very effective by itself, but it helped me keep my tongue flat so it wouldn’t pop out of the GMSS. This rig forced me to be a nose breather too. However, the nasal dilator did not open the nasal passages as well as PAP with a nasal/pillow mask.
The catches were: I had to sleep with my face on a rag or hand towel because of the drooling, it still didn’t solve the whole problem, it was hard to sleep with, and I rarely kept all that stuff in all night. I usually had to look for it each morning.
The pictures in this thread look like a knockoff of the Aveo, which I tried, along with another knockoff, but I liked the GMSS better.
While tongue advancement did have merit in principle, for me, it just wasn’t practical or ... enough. If it had been, I wouldn’t be using the Vauto now.
My father was an early CPAP user, with obstructive and central apnea. As a widower dating again at 48, I was very resistant to chaining myself to the big noisy boat anchor. As long as I was careful to be the last one asleep, I could slip all the plastic in discretely, and it all fit in the pocket-sized Z-Quiet case.
Long story short, my fiancée, a Registered Nurse, told me “it’s time” to give up on that stuff and start PAP. She was right. In return, I made her drag her Respironics out of the closet, looked at her data (bad), and gave her a Resmed For Her.
