Can delivered pressure be less that indicated?

Post by Garinda » Tue Oct 22, 2019 8:40 am

New to the forum. I've been using a ResMed Airsense 10 Autoset for about 6 months now. Everything was going along great until about 2 weeks. ago. The very first night I used this thing, I slept 9+ hrs (from a norm of 3-4). Then suddenly I started having trouble getting through the night. I started feeling like I was laboring to breath with it on. At the same time, my leaks consistently went to zero. In the previous months, I maybe had 3-5 nights of zero leaks. No almost every night is a zero leak. Events have gone from around 0.5 per hour to around 2-4 per hour. It sort of feels as if the pressure is down a bit even though it still indicates an 8. Settings: mode - CPAP, Pressure 8, Ramp - On, Ramp start - 4, EPR - On Full Time, EPR setting - 3. We have a vacation house in the mountains of Colorado. Elevation there is 8,650 ft. Elevation is 3,000 ft at our residence. This first happened after being at the vacation house for 3 nights. Still having the problem after returning home. Has anyone heard these getting out of whack where the delivered pressure is down while still indicating it is good? I've run mask seal and don't even notice that much difference between normal and mask seal. Secondary question: Does an elevation change of 5,500ft require an adjustment to pressure? Thanks for any advice.

You have an advanced machine. Why are you not using the auto feature?

I suggest you change the pressure settings to APAP min 8 max 12 as a start. You also might want to change the starting ramp pressure to 6.

The machines adjust to elevation changes automatically. They have been thoroughly tested up to 9500 feet elevation.

What is the breakdown of your events? I ask this because some people experience central apneas at higher elevations.

most anything can fail. Only way to be sure is to use a manometer, I've got a digital one from ebay, less than 30$

Yes, it's a machine, machines can fail, that said it's unlikely. check it with Oscar, and a manometer.

Also too much leakage can cause treatment pressure to be low, as can guessing what is going on. Jim

Unless the machine is malfunctioning, it will deliver at least the min pressure except possibly during the ramping period.

BTW, please fill in your equipment profile. That way, it will be inter every posting you make, and we won't have to find the message that has it.

I've got this great 4x4 vehicle, but I run it in 2 wheel drive. I keep getting stuck in the snow and mud. Any suggestions?

Yeah, put it into 4x4 and let's see how you do.

I've got this great APAP device, but I run it in CPAP mode. I keep getting poor results. Any suggestions?

Yeah, put it into APAP mode and let's see how you do.

I suggest:
Mode: APAP
Minimum 8, Maximum 20 (consider lower than 20 if you're not comfortable potentially going higher)
Ramp: Consider increasing. You posted you're feeling air stared and not enough pressure.
Leave other settings the same.

Then post some graphs.

Right now the machine might be trying to increase pressure, but you're not letting it. A few nights and we'll know if you need more pressure.

thanks for the suggestions. the reason my settings are where they are is because those were the settings specified to the supplier by the sleep lab dr. she also lectured me about not changing the settings. until very recently i didnt even know how to do that. this is because the thing was working so great for me as it was. all of that being said im not unwilling to change the settings. i just dont want to mess up my insurance paying for this. i've been told i must use it 4 hrs per night or insurance will stop paying. do they get this info from the resmed download site? or does the dr. report that to them. ill get a manometer and try the apap settings.

No.
A change of 5500ft may not require a pressure change but it may cause you to have an increase in CA's affecting your sleep quality

To get your data from the machine to oscar you use the sd card in your machine. If your machine didn't come with an sd card you can buy them just about anywhere. Amazon, Target, Walgreens.... I bought mine at CVS for less than $20. You can find them a cheaper. I went for convenience. CVS is close to my house. You don't need an expensive or large one just get the cheapest you can find.

I was talking to another shopper at Costco a couple weeks ago, who said he'd put out some skeletons, dressed up in pilgrim regalia, getting presents from a skeleton santa... and now he doesn't have to mess with his yard decorations until NEW YEARS! :D
