Mask too tight. Mandibular nerve irritated..mild facial paralysis

Post by JBarizona » Tue Feb 19, 2019 4:34 pm

Pressures are 13/20. At these higher pressures my resmed f20 fullface mask leaks too much if I don't have the straps real tight. I can live with high leak numbers, but I can't live with being woken up frequently every night by the noisy leaks blowing in my face. Problem is, the tight straps have irritated my mandibular nerve to the point where I'm noticing a mild paralysis on the right side of my lips. Sort of like novacaine wearing off after the dentist. Granted, its not the physical numbness of novacaine that I'm experiencing, but instead, the lack of motion/mobility in my lips on my right side. The lack of motion is subtle, but noticeable and I worry it will get worse.

Recommendations for a good FF mask for people that need high pressures like myself?

Recommendation for mask liners that actually work to minimize/prevent leaks for high pressure folks like myself? Do they work?

Any other recommendations that will allow me to loosen my straps while avoiding being woken by the frequent leaks.

Thanks in advance for any help.

JB

Adjust mask correctly or get one that fits. You can't live with excessive leaking, it makes your treatment not work. There are limits the machine can overcome. Jim

Thanks Jim. I have adjusted the mask dozens and dozens of times and that does not work. Can you recommend a mask or a type of mask liner that is known for alleviating leakage?

Pressures are 13/20.
I have to use a FFM, and I started out with a similar pressure range (12/20). My pressure would range as high as 18 or 20. I started using a firm foam cervical collar and eventually lowered my pressure to 8/20. Now, the machine rarely takes the pressure above 12 or 13.

The cervical collar holds the neck in the best position for airway patency and helps prevent jaw drop.

It would be cheap to give a cervical collar a good try. I recommend starting with a 3.5-inch width in firm. I'm a small person and use a 4-inch firm. https://www.walmart.com/search/?query=d ... l%20collar

I use this same mask and also have this same issue. Did you find a solution?

Rule of thumb: If a masks hurts--get rid of it.
If it hurts a lot, burn the damn thing.

I don't know about leakage, I use : https://www.cpap.com/productpage/pr-com ... -cpap-mask and https://www.cpap.com/productpage/TrueBl ... h-Headgear
I use both Flavors, mostly Nasal, but I do tend to mouthbreath, I have learned how to control it by holding my tongue to the roof of my mouth behind the top teeth, sucking creates a seal, after much practice, the brain allows this to work while you sleep. I check the leak data with software and if it gets too high, I use the FF mask, Now days, I mostly use the Nasal, however the FF seems to be cooler. In my 14 years I have tried two other masks, they did not make the grade.

Also fit the mask at full max pressure, laying down, notoverly tight or too loose. (Goldilocks Setting)!

Consider Resmed F20. Same shape as the F30 but has memory foam for the seal. It uses the same headgear, the masks are interchangeable. Using the one with the memory foam seal changes the name of the whole thing to "Air Touch F20." If you click on the equipment in my signature line you'll see it.

That has helped me with leaks - let me say I know exactly how you feel about leaks waking you up - and this one has really helped. I also go to high pressures in the teens and even hit 20 once.

There are two F20 cushions - the AirFit (silicone seal) and the AirTouch (memory foam seal). The F30 is a hybrid mask that is lower profile than the F20s, but still covering your mouth. The two F20s use the same headgear, but the F30 requires a different one.

I switched to the AirTouch about a week ago and really like it so far.

The problem with full face masks is they are really hard to seal because there's so much surface area to seal and your face moves all night. Is there any possibility you could get by with a nasal mask--perhaps using taping or a soft cervical collar to keep your mouth shut?

You might take a look at the Dream Wear FF mask. I have not tried it personally, but it looks like it would perhaps not need to be so tight (I could be wrong!).

I gave up on full face masks.
Like many other products, they are designed for the most common facial metrics.
If they want me to buy one, they need to build one that FITS.
