Hi all...haven't been on here for a while. Hoping someone may have some suggestions:
I was diagnosed with severe OSA last year and have been on CPAP since (Resmed S9 on manual setting of 8.6) Things improved somewhat but I was still very sleepy during the day.
Went in for another sleep study & MSLT but unfortunately didn't get enough sleep to do the MSLT, however they discovered I also have RLS and PLMD so was put on meds which again helped with the annoying legs at night....but still have EDS.
Just went back for yet another sleep study & MSLT & despite being really sleepy I was really restless and only got 4+ hours sleep so once again they had to abandon the MSLT.
Not sure whether they will go for a 3rd try. In the meantime I am really struggling during the day...wake up super tired every morning even when I get 10 hours sleep & could nap every day if given the chance. Foggy brain, difficulty concentrating, craving sleep. Also really concerned that I could doze off when driving in peak hour traffic.
I feel really embarrassed and annoyed I couldn't sleep enough to do the MSLT.
Anyone else have the same problem?? Any ideas for how to relax in a hospital environment as obviously sleeping tablets would defeat the whole purpose??
Other prob I had during the study,,,they use the Respironics with C-Flex machines whereas at home I use the Resmed with EPR. I think the machine was part of the reason I couldn't sleep as it felt weird when exhaling & I kept waking feeling like I couldn't breath. The nurse said every time it looked like I was going into REM I would wake abruptly & I think this may have been why. Seems really weird as it was on the same settings as my Resemed....any ideas???

During both my sleep studies, I took am Ambien. They told me that it wouldn't affect my results and I slept both nights for at least 6 hrs.

I had read that some few do not respond as well to the algorhythm of one manufacture as to another. I talked to a Resmed rep several times about my sleep problems and finally asked him what sleep labs in my area used Resmeds for titrations. Long story short he offered to supply Resmed equipment if my sleep lab was agreeable. I was pretty sure they were since they were the ones that suggested I be provided w/a Resmed rather than a Respironics due to my desire to be able to easily monitor the data.
So the Resmed rep and my sleep lab put their heads together and came up w/a date convenient for both and I went in for the Resmed titration. *sigh* I slept less than 2 1/2 hours out of 6.
Several years earlier for my initial titration I only slept 48 minutes out of 6 hours bedtime so they brought me back in for a second titration attempt - and I only slept 92 minutes out of 6 hours bedtime. *sigh*
In between the above titrations I had two other lab titrations where I slept sufficient time during each, one at my usual sleep lab and the second at another sleep lab.
My own personal theory is that altho tired, especially that initial titration and even the second, I was just too excited that now I was going to get the answer to gaining restful, restorative sleep! And for the Resmed titration I was again excited that my half-hearted response to PAP therapy was due to the different algorhythms and NOW I was finally get to that final step of absolute restful, restorative sleep. *sigh*

Sleep Study 1 - asleep in < 5 minutes, slept 8 hours

Multiple Sleep Latency Test (4 naps during the day, 8am, 10am, noon, 2pm) - average time was 3 minutes to fall asleep, slept 20-30mins each time
Sleep Study 3 (titration) - flipped and flopped for 2 hours, slept very little
I got just enough sleep during the titration for the doc to prescribe a cpap.
The result, the EDS is largely gone. No longer take daytime naps, no longer doze off when driving longer than 20 minutes.
Keep trying, maybe look into taking a sleep aid before the study and maybe look into some meditation/calming techniques, it sounds like it might be anxiety keeping you awake. It is a great feeling when the fog lifts and you feel alive again.

I also take 10 mg of Zolpidem (generic Ambien) if I can't fall asleep at the sleep studies within 30 min after lights out. Of Course with their permission. As to the RES (Residual Excessive Sleepiness) during the daytime I also have it in spite of 7 hours nightly sleep with good results on my APAP, since I started CPAPing 18 months ago. I have the RES only between after breakfast and noon. According to a recent French multi centers survey 12% of those on CPAPs with optimal treatment, suffer from this RES during day times.

1) the difference between RES and EDS is that RES refers to those who been XPAPing before while EDS does not necessarily.
2) The sleep specialist Dr Barry Krakow wrote that 60 min of sleep is enough to come up with a diagnosis or titration.

I get up around 6:00 A.M. for work 5 days a week;
but I can easily sleep until 11:00 A.M. on the weekend.

Deprive yourself of sleep for about 20 hours before arrival. If it is indeed anxiety, get a script for Xanax to take upon arrival. You will be out in 30 minutes tops.

just finished my titration study about 2 hours ago. took an ambian asleep < 10 minutes asleep 49 minutes up rest of night. no fun anywhere in sight
