AHI numbers getting higher.

Can't figure it out, I been on CPAP for over twenty years. Started out with a S7, then to the Phillips line, now on the Dream Station auto Bi-pap. my start numbers are 8 and 5. With use they are 13 and 10. My ahi for most nights are any where from a high of 9.0 to a low of 1.0. I use to never get over a 2.4. Yes an older now still getting 7+ hours sleep, but late afternoon I start to get sleepy. Anybody got an idea about this. My doctor is not worry about this, I am. :?

Are you using software to see what that AHI is made up of?

Can you post an example of a night with high AHI and nice low AHI?
So we can see if anything stands out that might explain the difference in AHI and how you feel.

Also, don't take this wrong, but are you on any (relatively) new meds? Have you (maybe slowly over time) gained any weight? Even a little can make a difference.

To all thanks for the help. Sorry for the slow reply. No new meds, my weigh 235-250, I'm on a yo-yo diet. Still trying to upload images.

For some reason or other your pressure needs are higher during some parts of the night and not others.
When this happens we normally look at 2 of the usual suspects....sleeping on your back or REM stage sleep both of which it is common to see our OSA worsen and/or pressure needs increase.

Your minimum EPAP isn't doing a good job holding the airway open and the machine can't respond quickly enough to get to where it needs to be when those whatever (supine sleeping or REM sleep) changes are happening.

You need more minimum EPAP. 7 or 8 would be what I would look at.
Even your "good" night isn't a great night IMHO.

Thanks for the help, will change the min should I also change the other? Right now on auto is 5 & 8. I would like to stay on auto if possible.

I would only change that minimum EPAP.
When you change the minimum EPAP it automatically will change the IPAP minimum and I see no reason to change the max IPAP at this time.
Don't blame you for wanting to stay with auto mode. That's my preference also.

Hey Pugsy, thanks for the help and all you do. I checked with my better half and found out I was sleeping more on my back. Now back on my side and my numbers are dropping. I will still raise my number a little bit. :D :) :)
