My sleep study results

Post by bugo » Fri Feb 02, 2018 2:29 am

I had a sleep study done a little over a year ago, and I've been using a CPAP machine since. It helps but it isn't a cure. On some nights my AHI is less than 1 and on some nights it gets as high as 30 or 40. Here are a couple of screenshots from my sleep study. Should I be concerned about having 21 central apneas in a night?

The study showed a bit of PLMD with low severity apnea, You had one bad desaturation during the night. You need to put some sleepyhead charts up, on a good night and a bad night. The forum may see something to help you in the charts, or like a change in meds or something. Otherwise it's back to the doctor, for a sleep study to see what has changed.

Do you have a copy of the titration study, where they determined your pressure needs on the machine? I would be interested in knowing if your limb movements increased during the period of time at the effective pressure. While that alone means nothing definitive, it can be one clue as to if limb movements are likely to still be a noteworthy issue. A few things stand out about your report. Some of your sleep stages were reduced so it's hard to be able to definitively say your worst case scenario was captured. The pages you've shown don't give a breakdown of sleep positions and how many of your events happened in which positions. It is generally thought that supine in REM in presents the greatest opportunity for apnea events to manifest. While your results are not uncommon for a diagnostic study, they have to be taken with an awareness that it may or may not tell your whole story. Had you had more REM and if that REM was supine, and if your few limb movements in the other stages happened to preclude apnea events during this study, your actual apnea account could be higher. On the flip side of that, I can't help but wonder if some of the apnea events precluded some limb movements. If so, treating the apnea theoretically could mean better sleep, which could allow more limb movements to manifest, meaning your real movement count could be higher when using CPAP at therapeutic settings. Which brings us to letting some of the data folks here take a look at your machine data. Your should not be having high AHIs when on CPAP unless there's extinuating circumstances. (However, if I mask up but then engage in a lot of conversation it will show up in my data as umpteen events in my first hour.)

Start with getting your CPAP experience optimized. Others here will be of help with that. If there is anyone who observes you during sleep, ask them if you move about a lot, particularly your feet or legs. Or videotape yourself. If the answer is yes, we can talk in more detail about it. Good luck sorting through things. Hopefully with more consistent effectiveness of your CPAP treatment everything will settle down and the legs become a moot point. Good luck with things.

Here are four recent screenshots. Two were very good nights, one was a bad night, and one was an awful night.

I would zoom in on all those CA and OA to see how bad they look and do they really look different. Your sleep study did not show a significant CA problem.

So any idea what might have been different between the good nights and the bad nights?
Which kind of night are you seeing more of...good or bad?
Do you take any medications of any kind..if so what?

The bad nights are seemingly random. I am on several medications, including Ambien.
