Dorking with ASV Settings as a Test?

Post by Uncle Flapp » Fri Oct 20, 2017 1:26 pm

I am considering a little experiment. Sure, I could pay up the wazoo for a sleep study but why not dork with my machine and generate my own results. As I've posted in many a thread, I travel quite frequently. I'd live to get me one of them new-fangled card-deck-sized travel CPAPs but they don't make 'em for ASV users. My question - what would happen if I tried APAP now that I am a couple years into therapy?

While using ASV, my central apnea index is non-existent. Had maybe 1 event in the last 30 days.
PTBs average 88% to 92%. Of those assisted breaths, I wonder how many are to treat transitional events or just erratic breathing in REM.

In hopes of still getting decent sleep with a dedicated (tiny) travel machine, I am curious as to what my AHI would be in CPAP mode. 95% EPAP averages 9cm so I don't need a ton of pressure. Sure, I'd still need to get used to life without bi-level pressure support.

What do you think - am I off my rocker?

- Flappy

edit, I just put my s9 asv into cpap mode and I couldn't get it to FOT to see if my airway was open or closed, so I may be full of it and made a mistake.

My goal is to see how I would fare with CPAP and possible invest in a small travel model. I don't believe they make mini BiPAPs.

Which numbers, PTBs?

It would be good to know what cpap pressure takes care of most or all of your obstructive apneas while minimizing central apneas. You may already know that number from prior cpap titrations before switching to ASV, but it may have changed. Mine is about 9.2.

Have to admit that the thought of that pre-ASV tiredness makes me cringe.

Nope, not off your rocker. But, do check the data after the first night, so if it's really not good for you, you won't be subjecting yourself to much of it.

I had the same thing happen with the AirCurve 10 ST that I had. I put it in cpap mode but it still put the obstructive apneas and central apneas in the same basket because FOT isn't used. Everything was in the unknown/unclassified basket and only hyponeas were separate.

It doesn't offer EPR in cpap mode either.

So while it is cpap mode and fixed single pressure...it doesn't do what the Elite does in terms of EPR and using FOT.
I was surprised to find this out. I thought it would work just like the Elite worked but it doesn't when it comes to FOT use and EPR.

FOT Forced Occillation Technique used by ResMed machines to help it figure out if the airway is open (central) or closed (obstructive).

Your DreamStation machine uses something different call pressure pulses to help it decide.
