I've been using cpap for 8 Monts. i have great results. amazing energy. im Bicycle miles upon miles. I've never been this fit. i. its just this one little problem. Jaw pain! excruciating pain. so bad i can`t stop crying or sleep.
i started clenching when i started my cpap so hard my teeth ache so bad. so i got a bite guard. helped with some of the pain. Tried various masks. nothing work. I've been thinking about a dental devise. i am a big girl. 210 pounds. will the devise help if i have to much With around my neck. and i have gotten tmj from the cpap and will the devise make it better or worse.

This sounds like you have a badly misfit mask. It's also a bit unusual to have this much pain without it causing additional disturbance to your sleep.

What mask are you using? How tight to you wear it? Are there straps that go near your TMJ?

Over the counter or custom made guard? If you got an OTC one, I would strongly suggest having your dentist make a custom made TMJ guard.

You have two questions here: (1) Will an oral OSA appliance work? and (2)Will an oral OSA appliance make the TMJ worse?

Will an oral OSA appliance work in the sense of treating your OSA? Maybe, maybe not. Oral appliances are usually NOT as effective at treating OSA as CPAP, but if your apnea is mild or if your OSA problem is caused by your tongue falling in the airway rather than your upper palatte falling into the airway, an oral appliance may work sufficiently well for you. Or not. Results with oral appliances are quite variable. Most dentists who fit them say the oral appliance is "successful" if using it cuts your AHI in half. That's a far weaker standard than CPAP is held to---CPAP is supposed to get your treated AHI under 5.0 regardless of how severe your OSA is.

Will an oral OSA appliance make the TMJ problems worse? Quite likely it will make them worse unless the dentist who fits is very, very careful AND you are lucky enough to only need a minor repositioning to prevent the apneas from occuring. The oral appliances work by forcing your jaw out of its normal alignment, and that puts stress on the TMJ. Getting evaluated by a dentist who both specializes in treating TMJ and also prescribes oral OSA appliances would be your best bet in trying to determine whether an OSA appliance will aggravate your TMJ problems. But finding such a dentist can be difficult, and they often do not take standard health or dental insurance. So the cost may be 100% out of pocket.

I started clenching the teeth out of the bleu (years ago and still do), it produced very bad teeth pain. At that time I had an APAP which changed pressure on an abrupt way. I changed machine's brand for one more slow to make the switch in pressure and it helped for a time but I started again. I did set it at CPAP mode and it helped again but then all started again and now I use a mouthguard made by the dentist. This fixed my problem. No more pain even if I use the machine as APAP. some times I feel a very little pain but that's all.

It sounds like it is pressure in the jaw, you may be tightening the mask too much? it should just be snug. There are youtube videos on cpap mask fitting.

you might like this heat and bite mouthguard, after fitting it has a centerpiece you can take out and breath through mouth if needed, that and a chin strap should help, total cost $5.

For someone who already has TMJ, I don't think I would recommend an OTC mouthguard.
But that is just me.
To the. OP, many masks fit best when the headgear is as loose as possible,
as air needs to fully inflate the mask, without wrinkles.
Start with the mask too loose and then carefully adjust until it is as loose it can be while fully inflated.
