Need some HELP - SleepyHead Charts - Excessive Tiredness

Let me give facts: (I wrote some of this in a past post)

So I've been using my Resmed Autoset 10, for approx. 7 months now, 100% compliance.

Full face Resmed mask.

I'm still feeling exhausted, and completely fatigued during the day. Naps are a necessity throughout the day. Headaches, weight is going up, BP is going up, etc.

Some background:

I did an at home sleep study, which indicated a lot of hypopnea's, AHI of around 9, Oxy. sat. dropping to low 80%. My sleep doctor started treating me with the above machine in APAP mode. In addition, my sleep phase cycle is off. Don't fall asleep until 2-3 am, and wake-up, 8 am. Avg. hours of machine use is about 5:30hrs.

I met with sleep doc about 3 months ago, as my symptoms are still terrible. He ordered a hospital c-pap study, wore my mask, their machine, in C-PAP mode. Hooked up to all the monitors including leg movement, etc. The study showed that at a pressure of 9, I got ~84 min. of Rem sleep.

About a month after meeting with him, my machine (wireless) was set with different settings (no one informed me ...still pissed about this). Now my machine has a ramp of 4, and max pressure of 9. Still no improvement .

Met with Sleep Doc. about a month ago. He's calling me an "interesting case." He wanted me to try provigal. I take a SSRI's and Benzos, etc. for severe anxiety and depression. Insurance did not approve though. So now we are trying light therapy to reset phase, and anytime I nap, I have to use the machine.

Still no improvement. Also, I now snore loudly while using the machine ... don't know why. My guess, is because I'm not getting enough pressure in the current setting. Also, I feel like I'm being suffocated when I first put the mask on, like I'm not getting enough air ...

Here are my sleepyhead reports for the 4 past nights ... Thoughts????

I would suggest increasing the minimum and maximum pressures to 10 cm. for about a week and see how that goes.
Also, I would recommend doing away with the Ramp and lower starting pressures.
Keep it in APAP mode ( essentially CPAP mode - min and max the same) as your flow limitations and snores would send your pressures to however high your maximum pressure would be.......and you're not really having all that many apneas or hypopneas.

If you're not already, try doing some nasal cleansing before bedtime (if you have some nasal congestion) and see if the snoring numbers diminish.

PS. If you'll notice, the nights you slept longer, your AHI was lower. So, try to get as much sleep as you possibly can. You need to repay your "sleep debt".

I would try a leak strap to reduce your leaks. Also, benzos and SSRIs can make people pretty tired.

Then, get familiar with YOUR machine and the settings........and take control of YOUR own therapy.
Nobody is going to care more about YOUR therapy than YOU will.
