New AVAPS user still with problems

Post by jmayer180 » Sat Oct 08, 2016 9:01 am

I’m not sure what is the best way for continuing long threads/revisions/updates. It seems to me that “Reply To” threads soon get so cumbersome and bulky as to be very hard to follow. I’ll try to summarize and update my situation.

After a follow-up sleep study I finally got my new PR System One BiPAP AVAPS. With my previous BiPAP Auto I had been getting CA's about double my OA's with total AHI's in the twenties, and with IPAP 25, EPAP 20 (or as close as could be tolerated) with restless nights and sleepy days. My new prescription is for AVAPS, PC mode, tidal volume 800 (I'm 6ft 3in, and weigh far, far more than ideal weight), back-up rate 12, IPAP max 25, IPAP min 10, EPAP 10. I used the new AVAPS machine for the first time 4 Oct 2016. Subjectively I felt great, with comfortable breathing all night. Imagine my surprise when I ran the data and found my total AHI was 45!

5 Oct 16, was essentially the same.

On 6 Oct 2016, I tried increasing my pressures a bit:

Last night, 7 Oct 2016, I increased the pressures to basically what they had been on my old BiPAP AUTO, with what seems to be dramatic decrease in my OA’s but still high CA’s and higher hypopneas.

There was some comment about whether SleepyHead accurately handled the PR System One BiPAP AVAPS data. FWIW, my Encore Basic shows essentially the same results.

I don’t understand why the AVAPS doesn’t work its magic and stop the CA’s, nor why now my events seem to be mostly hypopneas. Is that a good thing?

I’d be happy to have my sleep doc recommend changes, but my follow-up appointment is weeks away. I do not want to get too adventuresome without adult supervision, but are there any settings other than pressures that I could try?

Thanks to everyone for their patience and help.

How come they didn't give you the model 960 machine which could auto adjust as needed to both the obstructive apnea stuff and the central apnea stuff?
Do you have any sort of cardiac function issues? Congestive heart failure?

I don't know enough about how AVAPS work to offer any setting changing advice. The current settings obviously aren't working but I don't know how to fix it.

Staying with one thread allows the rest of us to follow your story rather than having to run around (which most won't) looking for the history so as to make some kind of sense of things.

it really is better to not make new threads every time, because that makes it harder for people who try to help you to reference back to what's been said, what's been tried, etc.

what are all the settings in the machine at this point?
