? higher pressure w/ Hybrid

Post by inacpapfog » Tue Aug 22, 2006 4:26 pm

Would obstruction of the Hybrid's exhaust vent cause higher pressure on an autopap?
Seems like the machine may interpret this occurence as an obstructing airway and would in turn start jacking up the pressure.

Do you mean a constant obstruction as in plugging up a portion of the exhaust flow throughout a sleep session or an obstruction that occurs momentarily and then is no longer obstructed as in rolling over face into your pillow for a while then rolling back off?

I think the APAPs make their interpretations on "relative" pressure changes. Thus if it is a constant obstruction, the APAP is not likely to notice any difference and if it is monmentary then it would pick it up.

Just my logical hypothesis not knowing a whole lot about this stuff.

I was thinking more of the temporary kind like you described, DreamStalker.
I think that sometimes the Hybrid's hose may get shifted upwards and partially obstruct the exhaust. I was just wondering if such things trigger pressure rises?

The exhaust flow is directly proportional to the surface area of the ports (ie small holes in the mask) and the pressure setting of your machine. In order to significantly affect the pressure on the inside of the mask, you would have to completely block the flow from any one or more of the port holes. The hose elbow positioned in front of the port holes is not sufficient for such an effect IMO and thus will have no effect on triggering the machine to raise pressure to prevent a false apnea.

What you may be observing is that the machine does work harder in order to make up for the high exhaust (leak) rate of the Hybrid because of the proportionality I mentioned above ... something I definitely noticed when going from thr Swift to the Hybrid. But the pressure between the inside of the mask and your lungs remains at what ever your machine is set to.

I hope that makes sense ... it is getting past my bed time and the my APAP is calling me now ...

Here's what I'm seeing: at roughly similar AHI's, the Hybrid results in substantially higher pressures (recorded by EncorePro) compared to pressures when using the CF2 or UMFF. At a setting of 14-18 on a Respironics Auto, the Hybrid runs pegged high most of the night, and the CF2 never touches 18. This is not a good thing, AFAIK, and does not seem to jive with other's experience.

Could this be from actual mask leaks (not constant exhaust leak)? around the top part maybe directly under the middle of the nose or the corners of the cushion?

I am seeing just the opposite. I'm running about 1.5 lower with the Hybrid than with the Swift and chinstrap/Tape/mouthglue.
I am seeing lower AI's and HI's and even hot 0.0 for the first time, but on average run 1.0 ,1.5.

DITTO (what sidesleeper said)...

i suggest you call the manufacturer if you are concerned about this. My pressure, as reported on another thread, is running 4 cms below that on UMFF, and I feel horrendous during the day. I called the manufacturer, who said 1-2 cms lower is not unusual, but mine is. They're sending me a new shell with smaller exhalation ports to try to fix this. The customer service guy said the important thing is how you feel, rather than your numbers, but they seem willing to work with people to have a good experience.
