Cannot lower AHI

Post by lennyten » Sun Aug 20, 2006 8:14 am

I have tried various changes to my pressures, as suggested, but have not been able to get my AHI below the 6 -7 range.

The following is a summary of the various changes and related results:

Code: Select all

DATE	AHI	AI	HI	APNEA	MAX	MIN	95%	LEAK

8/11	15.0	3.1	11.9	1.2%	10	6.4	10.2	0.04
8/12	6.8	0.9	5.9	0.4%	12	6.4	11.0	0.06
8/13	8.0	1.5	6.5	0.5%	12.6	6.4	10.2	0.04
8/14	6.7	1.6	5.1	0.7%	11.6	6.4	10.2	0
8/15	10.3	1.6	8.7	0.6%	11.0	6.4	11.0	0.08
8/16	6.6	1.0	5.6	0.3%	11.4	6.4	10.4	0.22
8/17	8.6	1.4	7.2	0.5%	11.4	7.4	10.6	0.22
8/18	9.3	2.4	6.9	0.9%	11.6	7.0	10.4	0.20
8/19	8.4	1.5	6.9	0.6%	11.4	6.6	10.4	0.18
Sorry for the poor presentation, but I cannot get it in a better format.

Other info:
Mode - Autoset
Set Pressure - 7.0 cm H2O
Start pressure - 4.0 cm H2O
Ramp Time - 5 minutes
Maximum Ramp Time - 30 minutes
Settling time - 10 minutes
Maximum settling time - 30 minutes

Any suggestions for other things to do that might help will be appreciated.

Thanks.

Not sure about your machine ..
some with more expertise will probably chime in

but I think your pressure is to low.

I think your machine is an auto ..
is is set as a cpap or apap

you might want to raise your pressure by one
remember i am not a doctor or have any expertise

follow any instructions knowing that

any thoughts from the more experienced???

I agree. my sleep study results said I did well on a setting of 7. I didn't feel good at this setting, I felt like I wasn't getting enough air. I raised the setting to 8 and felt great and haven't looked back since. pat

If the starting pressure is only 4.0 raise it to 6.0. A setting of 4.0 to start is really too low...as I found out.

I agree with wolftracker.
Another thing..... Is the mask setting in your machine correct?

If it were me....
Raise pressure minimum and maximums (if left in auto mode) and consider narrowing the range.
Experiment with fixed pressure and EPR settings. (Auto mode isn't ALWAYS the best).

I don't really know anything about the "Settling time", so I'll let someone else comment on that one if they think any adjustments might be necessary.

As is characteristic with ResMed machiens, most of your AHI is accounted for by hypopneas, not apneas,

In order to make sure that your machine is not scoring artifactual hypopneas during your periods of wakefulness before you fall asleep, I'd increase your settling time to beyond the ten minutes that you have it set for (unless you KNOW that you consistently fall asleep within ten minutes of putting on the mask.

It also looks to me like you have the thing set in CPAP rather than AutoSet mode. You might waant to confirm that.

Hope this helps.

Actually, your data looks like you're in AutoSet mode. What is your min.and max. pressure set at, and what were you prescribed for a pressure?

The actual pressures have been between 6.4 and 12.6, and in between.

It is in AutoSet mode. I have tried the min and max pressures between 6.4 and 12.6, and in between.

Then, I would lower or raise the max pressure, depending on which direction the AHI went.

Thanks.

Again, what is your titrated pressure. and have you tried increasing the settling time??

I ran AHIs with that machine between 6 and 7 no matter what I tried until one day, I thought of enabling and setting "settling time" and the next day, my AHI dropped and stayed below 2.

The "code" tags make for a pretty good column display. There may be a better way, but this works.

Settings (redux)

Post by GoofyUT » Sun Aug 20, 2006 12:12 pm

There's something wrong cause there is no "Set" pressure in AutoSet mode (only min. and max.)

The settings of 4-20 that you were advised were the settings of the unknowing and the unfamiliar. Find your titrated pressure or ask your sleep doc what he/she would prescribe for you for a pressure, and then set the Auto range at +/- 2cm or +/- 3 cm from that titrated pressure, I my OPINION.

But let us know how you do in increasing the settling time.

Settling time

Post by GoofyUT » Sun Aug 20, 2006 12:39 pm

I'd suggest a settling time interval NO longer than it typically takes you to fall asleep, and no longer. That's because, if you wake up in the middle of the night for say, a bathroom trip, and turn the S8 off and then back on when yoiu get back into bed, it'll reinvoke a settling time and if this is long, you may fall right back to sleep but since the S8 is still settling, you'll be untreated.

I used 30 mins. myself.
