If appears you've been on cpap for 2 to 2 1/2 months with no perceptiple improvement. SDB effects most eevryone differently, some improve quickly others take time. I assume you've make many changes to your therapy (tweaking so to speak) over this time period, some good some not so good. Maybe its time to punt. Find a pressure thats comfortable and thats eliminates your apneas, use a mask that doesn't leak to much that your comfortable with and forget about looking at numbers for a while. Perfection will never be found and sometimes the persuit of perfection can have unintended effects. Let your body settle on something constant for a while, let it acclimate to it. The only sure thing here is if you go off the hose your apneas will re occur and you'll do damage to your self. Out of a lifetime of your body doing its thing 2 to 3 months is an awful short time to expect major (or minor for that matter) changes. Good luck to you, prayers are with you.

Secret, you have been so pro-active and curious, that by reading your posts, I've learned alot, along with you. I hope that your sleep doctor can put it all together for you, and come up with a diagnosis and a solution. Please let us know, we're cheering for you.

SAGirl, my sleep doc and cardio both told me when I was bitching about my slow recovery that some folks need up to 12 months of CPAP therapy before the bennies kick in...I was grateful I wasn't one of those folks. I'll take slow any day. 2.5 months isn't very long and as I recall you're still in the home titration phase...right? This isn't a quick fix sort of condition, SAGirl, it's for the long-term, in most cases, it's for life. Getting the challenges resolved takes persistence and patience. I'm sorry you're struggling.

Secret Agent Girl,

I apologize that I don't remember your responses to our previous discussions (and I don't remember whether you have a diagnosis of OSA or UARS). I DO remember that you will not under any circumstances tape your mouth shut, so I'm not going to suggest that.

I really think you should try a full face mask. I did not feel appreciably better with the Swift LT. My leak rates did not suggest an obvious problem (although maybe my Auto-PAP would just crank the pressure up when my mouth was open, I don't know... Are you still on Auto-PAP?). When I switched to the Quattro, a FF mask, I felt better (unfortunately I couldn't wear it long-term because of it beating the bridge of my nose to a pulp, so I'm starting over.)

Perhaps you should get a FF mask and try it? If it doesn't help, you'll know it's not the mask. If you don't try it, you'll never know for sure. (But do prophylactically treat the bridge of your nose, at least the first few nights until you establish that it won't hurt you.)

I don't think you can expect anymore out of a mask than "it is comfortable and leaks from zero to insignificant, so I would guess the problem won't be solved with a new mask.

If she were wearing a Full Face mask and it was comfortable and the leaks were zero, I would agree that it is not the mask. But with nasal pillows, I just don't think you can say that.

I said I would get back to this. Apologies for the delay.

First some questions:

Does secret agent boy say you leak air through your mouth?
Wake up with dry mouth? (Don't remember that complaint, but checking)
Are you sensitive to sounds?
Do you use any medication to help with sleep?

It's possible you may need medication such as Ambien CR to provide longer lasting, deeper sleep. Sounds as if the therapy is interfering with your normal sleep cycle. You might even need a follow up study. Sometimes as a person adjusts to using xPAP their pressure needs change.

Definitely sounds as if you need to talk with your sleep specialist about this. I'm definitely at a loss. Again, it appears something is interfering with normal sleep. The xPAP should help, but seems to be causing a problem. Perhaps the continuous pressure is causing more arousals than if you had BiPAP.

Using a mask incorrectly however, may actually increase the risk of transmission,
rather than reduce it.
