Not sure the sleep doctor has this one right!

Post by ww » Tue Mar 05, 2024 9:08 pm

I just took a new home sleep test and it seems to be equally as good as the one I took in a sleep lab in 2008 including sleep EEG for sleep staging, sleep onset and arousal data.Their summary:
The patient had 137 apneas. There were 22 obstructive apneas, no mixes apneas, 12 central apneas, and 103 hypopneas for an AHI of 15.2/hour. Supine 309.2 min (38.8% of sleep 19.5 AHI. Right 327.7 min 60.7% of sleep 12.3 AHI.
Sleep Efficiency. 87%. AHI 15.2/hr. ODI 13.6l/hr. Avg SpO2 93.1%. Highest SpO2 98%. Lowest SpO2 78%. Max HR 95 bpm. Min HR 51bpm. N1&N2 62.8%. N3 5.1%. REM sleep 32.1%. Loud snoring all three nights. Time in bed 616.7 min. 539.6 min test.
Significant Oxygen desaturation observed with a total of 13.8 minutes below 89%. SPO2 longest span <89% was 1.0 minute. Sleep stages of N1, N1, N3, and Rem Sleep observed per EEG lead signals. RERAs, and Clear Air Hypopneas and Clear Air Apneas present.

Dx Severe Obstructive Sleep Apnea and Hypersomnia with OSA.
The Doctor prescribed a CPAP Auto set for 7-14 cmH2O.

Unfortunately I have been trying these and many other settings on Auto CPAP for the past 15 years and when I get rid of the obstructive apneas, they are replaced by central apneas. Can someone help me with settings for a new ResMed AirCurve10 VAuto and give me a chance of getting rid of both obstructive and clear air apneas and let me get rested in 8 hours instead of 10-12 hours?

We can try to help but need a lot more information including some software reports.
Just how many centrals are we talking about here per hour average?

Do you know about the software available to evaluate things?

Hi pugsy. I registered on this board a long time ago, so have patience with me.

I now have a new Resmed Aircurve10 and had decided not to chase any more rainbows with the Resmed AirSense10 Auto although it is only about 1 year old. I have OSCAR and it works on both machines. I only have two nights of data, but tell me where to look and I will post the latest night.

lThanks.

I think I got the screen capture file, but it has more than the amount of graphs shown. Now how do you upload the screen capture file to my account ?

One thing he (gen non-specific) got wrong was

Dx Severe Obstructive Sleep Apnea ...
By definition, severe obstructive sleep apnea AHI > 30. Moderate obstructive sleep apnea AHI between 15 and 30. Mild obstructive sleep apnea AHI is between 5 and 15.

If you remove the central apneas your obstructive AHI is 13.9, or mild obstructive apnea.

Just sayin'.

Maybe OP's doctor has a different definition of "severe".
Then again, simple numbers may be weighted by comorbidities.

I finally got the attachment uploaded. Question 1: Why do I sleep so long? Question 2: Are there any better settings to reduce snore and obstructive apneas and not cause central apneas?

There's not enough central apneas on that report to worry about. Do you have other reports with a much higher number of CAs/Central Apneas?
It's normal to have an occasional central apnea anyway.

Do you take any medications of any kind? If so, what?
Do you have any other physical or mental health issues? If so, what?

Are you waking often during the night for some reason or other? If so, why?

you asked "why do I sleep so long?". now i'm not trying to be a smart aleck here, but it may be because your body needs it.

or are you saying you're sleeping longer now than at any other time in your life?
people say i'm self absorbed.

Great, then show some specific sleep-related examples.
