Severe mixed apnea at the age of 35

Hey guys,

I'm new to this board an hope you may share your experience.

I'm 35, male, generally healthy, not overweight or similar. Due to sudden afib episodes last year a few months after covid is was also screened for sleep apnea, despite I've never snored. Unfortunately the results turned out that I had severe mixed apnea. 2/3 of it were hypopneas and 1/3 central apneas. Nothing obstructive at all. The reason the sleep lab diagnosed mixed apnea was, that they recorded constant snoring, which was really strange to me. If you ask me, also my hypopneas are central.

Next day I was put on cpap and miraculously it helped with both, hypopneas and central apneas. While I had an AHI of 58 in my first night recording without therapy, under cpap it went down to an AHI of 2. This is the first strange thin to me: Why does cpap seem to help, while it usually shouldn't have an effect on the central apneas.

This was great news, considering the general diagnosis. Now what is still buggering me, is that when checking Oscar I found out I have periodic breathing every night. My Resmed 10 sometimes even marks it as CSR. Sometimes it shows the regular pattern with the periods and central apneas in between each period. sometimes it's only hypopneas in between or even milder. Looking through my data of each night, it seems as if I had some degree of periodic pattern underlying my breathing g most of the time, but mostly the ups and downs are so mild, that you only barely see it.

My heart and brain have been checked thoroughly including MRIs. Still I'm very concerned about what might be causing all this nonsense and if my therapy really fits.

IMO should sign up for the free SleepHQ and post full night data. I'm thinkin' you need a CS2.

But I don't have heart failure. My LVEF is at 69% of max. 70%.

They invented it for CSR but it didn't take them long to start using it for everything.

Is there any known cause for CSR besides heart failure?

Also did you have a look on my sleephq? what does it tell you?

ResMed overscores CSR. The cycle time looks too short to be CSR.

You do have lots of periodic breathing:



I'd be thinking CompSAS, but it's like only 7.0 cmH2O, and you've had a couple months to acclimatize...

Have to give this some thought.

What do you mean by "overscore"? Sorry, im not a native speaker.

Reg. comSAS: This would mean the central are induced by cpap itself, right? Sporadic periodic breathing was already mentioned in the report of my first night in the lab, without cpap. so it seems I usually have this, not only cause of cpap
