My BiPap is maxed out on pressure and AHI over 5... What's next?

Hello everyone. First time poster. I've been on BiPap for quite a few years and have been very compliant and love how well it works. I have a Resmed BiPap Auto and the Ipap pressure is now maxed out on 25cm with my AHI over 5. Anyone know that the typical treatment is for someone that is maxed out on BiPap and still having an AHI over 5?

I don't have Oscar and my apneas are not central, just obstructive.

So exactly what brand and model machine are you using and which mode and all the settings????

Usually it is EPAP that we look at for preventing obstructive apneas as doing the bulk of the work...IPAP just helps out.

Using OSCAR would help us see if there is a pattern to the obstructive events like maybe sleeping position or REM stage sleep or even chin tuck ....which might mean we could help reduce the need for higher pressure by altering the positioning of the airway a bit. Are those obstructive events happening in clusters and parts of the night without events....or are they happening on a regular basis all night long. That information is only available by using the software...get the software if at all possible and use it to stand the best chance of figuring out a solution to your problem.

I'm using a Resmed AirCurve 10 VAuto. I've always been a side sleeper and never sleep on my back. I use a Resmed F20 FFM with no leaks. My sleep report in the mornings show that the max pressure is 25 with AHIs over 5.

So that tells me essentially nothing. Doesn't tell me what you start out with and doesn't tell me what mode and doesn't tell me when it changes...if it changes.

If Oscar is a problem for you, use SleepHQ which can be accessed from any device with a browser, including phones and tablets. Upload your data there then post a link here. It's easier than Oscar but not as comprehensive.

Either way I agree with Pugsy that we need to see detailed data to be able to help you. Otherwise we're only guessing.

You need to both see and provide data. Otherwise we can only reasonably guess that your minimum EPAP is too low. What the machine itself reports on screen is largely useless.
