Hi there,

Sleep study showed Moderate Sleep Apnea, 23.8. Titration with 8cmH2O was able to get me down, I think, to 1 event per hour.

I'm struggling with the straight CPAP mode, which was set 4.0 ramping to 8.0. I find exhaling against that pressure pretty uncomfortable. I essentially couldn't sleep more than an hour or two at the most without waking up and needing to tear the mask off. I was able to get my events per hour down to around 3.9 --- although for only a couple hours of barely tolerable usage.

The EPR setting was set to level 3, full-time. Pulmonologist suggested using AutoSet instead of straight CPAP mode --- although they never succeeded in sending my medical supply company an order to change the settings.

I changed the setting to Autoset, with minimum pressure 7, maximum pressure 9. The AutoSet mode is much more tolerable, and while definitely not perfect sleep, I was able to sleep most of the night. I ended up with about 5:20 of usage, which is the most so far in a single night. However, I'm seeing 9.0 events per hour, which is admittedly down from my "normal" 23.8. I own a CMS50F pulse oximeter --- it's reporting around 15 events per hour without the CPAP.

I could really use some help optimizing the settings. My "Resmed myair" score was 77. I feel like I should be shooting for a lower events per hour.

Is it as simple as increasing the min/max pressure settings, doing the best I can with adherence, and minimizing the events per hour?

welcome to the zoo! yes, it can be just that easy. but the experts here are going to need to see some data from you to be able to make suggestions on just what those settings need to be.

please see my links below. grab the oscar software(it's more detailed than what you are currently using) and post some charts for review.

i must say you have an excellent attitude. you want to take charge of your therapy. and once you do that, things will improve.

  • The wrong machine is listed in your signature:
    You have either the Resmed Airsense 10 AutoSet or the Resmed
  • Airsense 10 AutoSet for Her
  • MyAir is a terrible diagnostic tool, use OSCAR

  • Ensure you have an SD Card in your machine
  • Upload to OSCAR
  • Post graphs
Ask if you need specific help for any of the above, and use this topic as your therapy thread.

Now people will have some concrete data to offer advice from.

I'd move the max setting to e.g. 15 or even 20 (most common) and see if it helps. It's the min. one that does the job and max can usually be left high unless there's a known reason not to.

I do know that I've been bumping my minimum/maximum autoset pressures and I was really hoping to see a corresponding AHI decreases, but I'm not. My original straight CPAP 8.0 setting seems to have yielded the lowest 3.9 AHI rating so far.

I got my second "free" Airtouch F20 in the mail today. So I've got the Dreamwear Full as listed in my signature and this one to try. No clue if they would affect effectiveness or if it's just a comfort thing.......

You don't make your decisions based on single evening sessions, you make them based on trends, or key markers in the charts. Our nightly session summaries are too volatile, especially when we are new, not sleeping well and have such short session.

There are some pinned topics that describe the prefferred format of the charts.

If the pie chart doesn't push stuff from the statistics off the page (like it's doing in Zonker's example), it's OK...

Hello, Keith! It’s great that you’ve started treatment for your apnea and that you are taking charge of getting it optimized. Plus you’re already using the software!

As soon as you can post a Daily View chart set up as Zonker has indicated, the experts can help you get your AHI down. You don’t need to chase any magic number, but for the full benefits of treatment to come your way, your AHI will definitely need to be lower. And it will be!
