Also, does your computer already have a smart card reader that could be causing conflict? For example, my dell laptop has a smart card slot which I use for authentication purposes. Its barely visible - a thin slot above the express card slot.

Dave, maybe. On the laptop, there is a card slot of some kind and a flash card slot for SDs. For some reason it does SDs Ok but not the higher density SDs. But that's on my Vista Laptop. I tried it on an XP desktop and a Windows 7 laptop. There may be some other answers from printer drivers, like the Canon D1150 I have that hooks up to the laptops via WiFi, the desktop through a USB and I have a large Epson r1900 photo printer going into the desktop through another usb.

I unistalled the driver from within the Device Manager of the Vista Laptap. I removed Rescan 3.12 rebooted and re-intalled it. The card reader after playing with it weorked once and then never again with either card. I wrote to ASC and told them and they said thay would get me right driver if I give them the serial #. I also told them that my Doctor wants me to have it and I have a script for it. I am not sure what would make it work one time and no another on a laptop that isn't doing anything but trying to get the card reader to work. Even the RT at the DME said that it doesn't always work for her either and she has 2 card readers, one older one and one with the R at the end of the product model number.
It appears the Resmed Reps give the RT at the DME the card readers and the software for free, they don't haveto buy them and that way they don't have any to sell to lowly patients who don't deserve decent therapy and might raise questions about how good or bad their software and or machines are. It is disgusting. But the preventing of DME's from selling the card readers and software to the patients who have scripts, which Resmed appears to have done, is not only crimnal but could create liability for Resmed in the case where someone was not sleeping well and that contributed to his otr her death for all the reasons we know about their could be liability for accidents caused by bad sleep of an apnea patient in therapy with unmonitorable Resmed equipment (assuming patient tried to monitor but was denied the ability to buy the software or hardware.

What about the green light? is it always doing something (blinking) with or without a card installed?

In reading through the post I was not able to find what sort of computer you are running on so I am not certain if this will apply, but....
When I first installed mine a couple of years back I also had big problems.
I finally found the solution on the ACS support site.
If you are using a Wireless network adapter on either a desktop or laptop, that can somehow interfere with the ACS driver installation.
Try disabling the wireless adapter during the installation. Once the card reader is installed you can turn the wireless back on. Somehow just stopped it from identifying the correct drivers.
I also used the drivers from ACS rather than the ones included with the software.

Your ire should be directed at the FDA, as they set the rules about what happens in your part of the world.

Got interested in this thread since I have a ResMed VPAP S in standby (backup) from which I had neither used nor downloaded info into ResScan. I have a reader that I ordered from the party in Israel and the card that came with the unit. Plugged the card into the machine and it reported copying had completed. I inserted the card into the reader and plugged the reader into a USB hub. Everything looked good; computer said drivers had been installed. Opened ResScan, created a new patient, and attempted to download anything that might be on the card into ResScan. Got the same msg the OP got; no flow generator available. Remembered seeing the suggestion above so I plugged the reader USB plug DIRECTLY into the computer. VOILA!! The computer reported that more drivers had been installed and everything now works as advertised.
